分离自烧伤患者耐药弗氏柠檬酸杆菌的耐药基因分析 被引量:1

摘要 目的分析分离自烧伤患者的2株耐药弗氏柠檬酸杆菌的耐药基因,了解菌株对B内酰胺类、氨基糖苷类、氟喹诺酮类药物的耐药机制。方法笔者单位烧伤科分别于2012年6、8月收治1例分离出弗氏柠檬酸杆菌的大面积烧伤患者(每例患者分离1株菌),应用药敏卡AST-GNl3检测分离出的菌株对阿米卡星、氨苄西林、氨苄西林/舒巴坦、氨曲南、头孢唑林、头孢吡肟、头孢替坦、头孢他啶、头孢曲松、环丙沙星、厄他培南、庆大霉素、亚胺培南/西司他丁、左氧氟沙星等20种抗生素的敏感性;采用PCR法检测35种B内酰胺酶编码基因、15种氨基糖苷类修饰酶编码基因、6种16s核糖体RNA甲基化酶编码基因、2种氟喹诺酮类药物作用靶位编码基因和3种氟喹诺酮类药物作用靶位保护蛋白编码基因的表达,选取阳性表达基因进行DNA测序。结果2株弗氏柠檬酸杆菌对检测的13种B内酰胺类抗菌药物均耐药,对3种氨基糖苷类抗菌药物均耐药,对2种氟喹诺酮类抗菌药物耐药。1株弗氏柠檬酸杆菌对复方磺胺甲嗯唑耐药,2株弗氏柠檬酸杆菌均对呋喃妥因敏感。2株弗氏柠檬酸杆菌检出blaTEM、blaSHV和blaIMP3种p内酰胺酶编码基因,检出aac(3)-Ⅱ和ooc(6)-Ⅰb2种氨基糖苷类修饰酶编码基因,检出氟喹诺酮类药物作用靶位DNA回旋酶及拓扑异构酶编码基因gyrA、parC。经DNA测序分析,菌株在氟喹诺酮耐药决定区序列存在有义突变,gyrA编码基因第83位密码子由丝氨酸突变为异亮氨酸,parC编码基因第80位密码子由丝氨酸突变为异亮氨酸。结论2株弗氏柠檬酸杆菌检出blaTEM、blaTEM和blaIMP3种D内酰胺酶编码基因以及acc(3)-Ⅱ。aac(6)-Ⅰb氨基糖苷类修饰酶编码基因,可解释其对全部B内酰胺类(含碳青霉烯类)药物、氨基糖苷类药物耐药的机制。氟喹诺酮耐药决定区存在有义突变是该2株弗氏柠檬酸杆菌对氟喹诺酮类药物耐药的遗传学基础。
