左房黏液瘤的X线与超声诊断(附10例分析)

X-ray and Echocardiogram Diagnosis of the Atrial Myxoma (Report 28 Cases)

目的 :探讨左房黏液瘤的X线平片诊断和超声诊断价值。材料与方法 :本组 10例 ,男性 4例 ,女性 6例。均采用心脏常规位片及超声心动图检查 ,并手术病理证实。结果 :10例左房黏液瘤的X线平片特点 :(1)左房增大与心脏增大不相一致。 (2 )左房、右室和肺动脉段的改变不成比例。 (3)左房增大与肺郁血、肺循环高压的程度不相称。超声心动图特点 :(1)收缩期心房内可见云雾状光团回声。 (2 )二尖瓣前叶呈城墙样改变 ,舒张期光团随血流移入二尖瓣口。结论 :根据左房黏液瘤的临床及X线表现特点 ,平片可以提出可能性诊断 ,但超声心动图是最精确、可靠、简便、安全的诊断。

Purpose:To explore the value of x-ray film and echocardiogram in diagnosis of left atrial myxoma(LAM).Materials and Methods:Ten patients of LAM,including 4 males and six females,were examined by cardiac series and echocardiogram.All of these cases were proved by operation and pathology.Results:The X-ray film signs of LAM were(1) the enlargement of left atrium didn′t compare with the cardiac silhouette enlargement.(2)The variations of left atrium,right ventricle and pulmonary arteries were not parelleled.(3)The enlargement of left atrium didn′t match (pulmonary veins obstruction)and pulmonary circulation hypentension.The echocardiogram features of LAM were(1)The cloudy-like echo mass was seen in left artium in systolic period.and the mass companied with the blood current shifted to mitral valve in diastole.(2)The anterior leaf of mitral valve presented wall-like change.Conclusion:The cardiac series could give a potential diagnosis of LAM according to its clinical and X-ray performances,but the echocardiogram might be the were accurate,reliable,simple and safe way.
