摘要
目的探讨遗忘型轻度认知损害(aMCI)与阿尔兹海默病(AD)患者及正常人的事件相关电位P300及其亚成分的特征。方法应用ERP-P300检测对阿尔兹海默病、aMCI和正常对照组各20名受试者进行认知功能评估,比较分析3组受试者P300及其亚成分的变化特征及组间差异。结果 AD组的P300潜伏期较aMCI组和正常对照组明显延长,波幅降低,差异有显著性(P<0.05),且与MMSE得分呈正相关;AD组,aMCI组和正常对照组比较,三者双峰出现率有统计学差异(P<0.01)。而aMCI组和正常对照组对比P300的潜伏期和波幅无差异性(P>0.05),而双峰出现率有统计学差异。进一步分析提示:AD和aMCI及正常组对照其P3a、P3b的潜伏期延长、波幅减低有统计学差异(P<0.01)。结论 P300及其亚波分析可以作为阿尔兹海默病及遗忘型轻度认知损害早期诊断的客观依据之一。
Objective To investigate amnestic mild cognitive impairment ( aMCI) ,patients with Alzheimer's disease ( AD) and normal event-related potential P300 and its sub-component characteristics .Methods Cognitive function was assessed in an AD group,an aMCI group and a normal control group ,each with 20 subjects, using ERP-P300.P300 of the three groups of subjects and its sub-components of variation were analyzed .Results The P300 latency of the AD group was significantly lon-ger than that of the aMCI group and the control group ,and the amplitude decreased(P〈0.05).There was a significant differ-ence and it was positively correlated with MMSE score .Bimodal occurrence rate of the three groups were significantly different (P〈0.01).The latency and amplitude of the P300 in the aMCI group and the normal control group were of no difference (P〉0.05),but there were significant differences in the bimodal occurrence rate peaks .Further analysis indicated that:AD group was statistically different in P3a and P3b's latency and amplitude reduction (P〈0.01).Conclusion P300 and its sub-wave length analysis can serve as an objective basis for early diagnosis for Alzheimer's disease and amnestic mild cognitive impair-ment .
