大剂量顺铂为主联合化疗对晚期非小细胞肺癌的疗效评价

EFFECTS OF THE EXTRA LARGE DOSE CISPLATIN COMBINATION CHEMOTHERAPY ON PATIENTS WITH ADVANCED NON-SMALL-CELL LUNG CANCER(NSCLS)

目的 :观察超大剂量顺铂为主的联合化疗对晚期非小细胞肺癌 (NSCLC)的临床疗效及毒副反应。方法 :38例晚期 (NSCLC)患者随机分为治疗组 (2 6例 )和对照组 (2 6例 ) ,两组均采用 CAP方案 [环磷酰胺(CTX)、阿霉素 (ADM)、顺铂 (PDD) ]。用法 :CTX 1 .0 g/m2 ,ADM 40 mg/m2 第 1天和第 8天给予 ,两组用量相同 ,每 4周 1次 ,共 3疗程。治疗组 PDD用量 50～ 60 mg/m2 ,总剂量为 72 0～ 1 0 80 mg,对照组 2 0～ 30mg/m2 ,总量 360～ 450 mg,分别于第 4、5、6天给予。结果 :治疗组 CR5/2 6,PR1 4/2 6,总有效率 73% ;对照组PR 1 0 /2 6,总有效率 39%。两组化疗后毒副反应相近。结论 :超大剂量顺铂为主的联合化疗可显著提高NSCLC的临床疗效。

Objective:The aim of this study is to evaluate the effect and toxicity of combined chemotherapy with extra large dose of cisplatin in patients with advanced non small cell lung cancer(NSCLS).Method:Thirty eight advanced NSCLS patients were divided into treatment group(n=26) and control group (n=26) randomly.All patients were treated by cisplatin(PDD) combined with cyclophosphamide(CTX) and doxorubicin(ADM).CTX was administered at a dose of 1 000 mg/m 2 on first and eighth day,ADM at 40 mg/m 2 on first and eighth day,and repeated every 4 weeks for 3 months for all patients;PDD at 50 60 mg/m 2 on consecutive three days,total 720 1 080 mg for treatment group and at 20 30 mg/m 2 on consecutive three days,total 360 450 mg for control group.Results:There are 5 patients with complete response(CR) and 14 patients with partial response,the total efficiency 73% in treatment group and 10 patients with partial response,total efficiency 39% in control group.The systemic toxicity is similar in two groups.Conclusions:Our results suggested that the extra large dose of cisplatin for combined chemotherapy can significantly improve the effects on patients with advanced NSCLC.
