人乳头状瘤病毒感染与乳腺癌病理分级及C-erbB-2的关系

Correlation between human papillomavirus infection and pathological grading of breast cancer and C-erbB-2

摘要 目的通过检测乳腺浸润性小叶癌组织中人乳头状瘤病毒(HPV)一DNA及癌蛋白C—erbB-2表达,分析HPV感染与乳腺浸润性小叶癌病理分级的关系及HPVDNA与C—erbB-2表达的相关性。方法选择2012年1月一2013年1月普外科诊治的乳腺浸润性小叶癌患者102例为研究对象,所有患者均接受手术治疗,取病变组织行病理学检查,检测HPV—DNA及C-erbB-2;分析病变组织中HPV—DNA与乳腺浸润性小叶癌病理分级的关系及HPV—DNA与C-erbB-2表达的相关性。结果102例乳腺浸润性小叶癌患者病理分期I~Ⅳ期中HPV阳性48例,阳性率为47.1%;不同分期患者HPV阳性比例差异无统计学意义;HPV一16RNA阳性率为47.1%,C—erbB-2阳性率为43.1%;I~Ⅳ期HPV一16RNA阳性率分别为33.3%、42.0%、58.3%和70.O%,呈上升趋势;I~Ⅳ期C—erbB-2阳性率分别为27.8%、40.O%、54.2%和60.0%,也呈上升趋势;在乳腺浸润性小叶癌中,HPV一16RNA与C—erbB-2表达呈显著正相关(r一6.43,P〈O.01)。结论乳腺浸润性小叶癌组织中HPV阳性率高,随病理分期的增加,HPV一16RNA阳性率及C—erbB-2阳性率均逐渐升高,且HPV一16RNA与C—erbB-2表达呈显著正相关。 OBJECTIVE To investigate the expression of HPV-DNA and C-erbB-2 in invasive breast lobular carcinoma and analyze the correlation between the HPV infection and pathological grading of invasive lobular breast carcinoma and the expression of HPV-DNA and C-erbB-2 expression. METHODS A total of 102 cases of invasive lobular carcinoma, who were treated in the department of general surgery from Jan 2012 to Jan 2013, were enrolled in the study, all the subjects received the surgical procedures, then the lesion tissues were sampled for the histopathological examination, the HPV-DNA and C erbB-2 were detected, and the correlation between the HPV-DNA in the lesion tissues and the pathological grading of invasive lobular carcinoma and the expression of HPV-DNA and &erhl3-2 was analyzed. RESULTS Among the 102 cases of invasive lobular carcinoma with the pathological grading ranging between [ and IV, there were 48 cases detected positive for HPV with the positive rate of 47.1 , the difference in the positive rate of HPV between the subjects with different pathological grades was not statistically significant; the positive rate of HPV-16RNA was 47.1 , and the positive rate of C-erbB-2 was 43.1; the positive rates of HPV16RNA of the subjects with the pathological grades ranging from I to IV were respectively 33.3, 42.0 ,58.3, and 70.0%, showing an upward trend the positive rates of C-erbB-2 of the subjects with the pathological grades ranging from I to IV were respectively 27.8%, 40.0, 54. 2,and 60.0, showing an upward trend. Among the subjects with invasive lobular carcinoma, the HPV-16RNA was positive correlated with the expression of C-erbB-2 (r= 6.43, P0.01). CONCLUSION The positive rate of HPV is high in the breast infiltrating lobular carcinoma tissues with the increase of pathological grading, both the positive rate of HPV-16RNA and the positive rate of C-erbB-2 show an upward trend, and there is a significant positive correlation between the HPV-16RNA and the expression of C-erbB-2.
