摘要 目的:探讨P波离散度(P wave dispersion,Pd)预测高血压患者阵发性心房颤动发生的价值。方法:将95例高血压患者按有无心房颤动分为两组。采用同步12导联体表心电图测量Pmax、Pmin和Pd,超声心动图测定左心房内径(LAD)和左心室射血分数(LVEF),比较两组间Pmax、Pmin、Pd、LAD和LVEF的差异。结果:观察组Pd、Pmax及LAD高于对照组(P<0.01,P<0.01,P<0.05),观察组LVEF低于对照组(P<0.05)。3个指标中以Pmax>110 ms+Pd≥40 ms的特异性和阳性预测值最高,而敏感性和阴性预测值最低。结论:Pd是高血压合并阵发性心房颤动的预测因子,与Pmax联合应用预测价值更高。 Objective:To investigate the value of P wave dispersion(Pd) in predicting a trial fibrillation in patients with paroxysmal hypertension. Method:A total of 95 cases with hypertensive patients were divided into two groups according to whether the patient had atrial fibrillation.Pmax,Pmin and Pd of patients were detected by synchronous 12 lead ECG,and at the same time left a trial diameter(LAD) and left ventricular ejection fraction(LVEF) were detected by ultrasonic cardiogram.The differences of Pmax,Pmin,Pd,LAD and LVEF between two groups were compared.Result:Pmax,Pd and LAD in the observation group were higher than that in the group(P&lt;0.01,P&lt;0.01,P&lt;0.05),and LVEF in the observation group was higher than that in the group(P&lt;0.05).The specificity and positive predictive value of Pmax&gt;110 ms+Pd≥40 ms were the highest in three indicators,and the sensitivity and negative predictive value of Pmax&gt;110 ms+Pd≥40 were the lowest.Conclusion:Pd is a predictor of hypertension combined with paroxysmal a trial fibrillation,and the predictive value of it combined with Pmax was higher.
