摘要
目的探讨CD56在急性白血病(AL)中的表达情况,分析CD56阳性急性髓系白血病(AML)和急性淋系白血病(ALL)患者的免疫表型特点。方法应用流式细胞术(FCM)检测76例急性白血病患者CD56的表达情况,分析CD56+AML和CD56+ALL的免疫表型特点。结果 CD56在AML患者中表达的比例为32.1%(18/56);在AML亚型中,CD56阳性患者在M1、M2、M3及M5中所占比例分别为33.3%(3/9)、47.6%(10/21)、9.1%(1/11)及28.6%(4/14)。CD56在ALL患者中表达的比例为30.0%(6/20);在T-ALL中,CD56阳性患者的比例为44.4%(4/9);10例B-ALL患者中有1例表达CD56,比例为10.0%;1例NK细胞白血病表达CD56。此外,与对照组相比,CD56+AML患者中CD33的表达率明显增加(P=0.021),而CD56+ALL患者中CD7的表达率亦明显增加(P=0.024)。结论伴CD56阳性的急性白血病细胞,其在各型白血病细胞中的免疫表型各有特点,多见于M2和T-ALL。且CD56+AML患者中CD33的表达率较高,CD56+ALL患者中CD7的表达率较高,研究结果将为CD56+AL患者的治疗和预后分析提供重要的指导意义。
Objective To investigate the expression of CD56 antigen in acute leukemia (AL),and to analyze the immune phenotype of CD56-positive acute myeloid leukemia (AML) and CD56-positive acute lymphoid leukemia (ALL).Methods The expression of CD56 in bone marrow of 76 AL patients was detected by flow cytometry(FCM).And the immune phenotype was analyzed in CD56-positive AML and CD56-positive ALL patients.Results The percentage of CD56 expression in AL patients was 32.1% (18/56).And in AML subtypes detected,the percentages of CD56 expression were 33.3%(3/9),47.0%(10/21),9.1%(1/11),and 28.6%(4/14) inM1,M2,M3andM5,respectively.The percentage of CD56 expression in ALL patients was 30.0% (6/20).In T-ALL subtype,the percentage of the CD56 expression was 44.4% (4/9),while the percentage of the CD56 expression was 10.0%(1/10) in B-ALL subtype.One case(1/1) of NK cell ALL was found to present CD56 expression.In addition,compared to respective control group,CD33 expression was significantly upregulated in CD56+AML patients(P=0.021),while CD7 was significantly upregulated in CD56+ALL patients (P=0.024).Conclusion The data suggest that the immunophenotype of AL patients with positively expressed CD56 is variable.And CD56 is more expressed in M2 and T-ALL patients.Moreover,in AML patients,CD56 expression is related to a significant expression of CD33,while in ALL patients,CD56 expression is associated with significant expression of CD7.Thus,this data contribute to the treatment and prognosis for acute leukemia.
