阵发性房颤与NT-proBNP水平及P波离散度的相关性研究 被引量:14

摘要 目的探讨阵发性心房颤动(房颤)与N末端脑钠肽(NT-proBNP)水平及P波离散度的相关性,为阵发性房颤患者的诊断、预后及治疗提供依据。方法选择2008年7月至2010年10月内蒙古自治区人民医院收治的37例阵发性房颤患者作为房颤组,同期健康体检的34例窦性心律患者作为体检组。分别测定两组患者的血清NT-proBNP水平和P波离散度,并进行统计分析。结果①房颤组房颤时NT-proBNP水平显著高于体检组(P<0.05);②房颤组转复为窦性心律后NT-proBNP水平仍显著高于体检组(P<0.05);③房颤时NT-proBNP水平高于转复为窦性心律后NT-proBNP水平(P<0.05);④房颤组转复为窦性心律后P波离散度与体检组P波离散度比较差异有统计学意义(P<0.05),房颤组P波离散度大于体检组;⑤房颤组转复后NT-proBNP水平与P波离散度呈显著性正相关(r=0.215)。结论检测患者血清NT-proBNP水平和P波离散度对房颤的诊断、预后有一定的指导意义,值得进一步研究及临床应用。 Objective To explore the correlation between N-terminal pro-B-type natriuretic peptide (NT-proBNP) level and P wave dispersion and paroxysmal atrial fibrillation (PAF), to provide references for diagnosis,prognosis and treatment of PAF. Methods Thirty-seven PAF patients admitted to Inner Mongolia Region People's Hospital from Jul. 2008 to Oct. 2010 were selected as the experimental group( PAF group). 34 volunteers with sinus rhythm taking medical examination during the same period were selected as the con- trol group. The serum NT-proBNP level and P wave dispersion of the two groups were tested and statistically analyzed. Results ①The NT-proBNP level of the PAFgroup during atrial fibrillation was significantly higher than the control group( P 〈0.05 ). ②The NT-proBNP level of PAF group after cardioversion was still signifi- cantly higher than the control group( P 〈 0.05 ). ③The NT-proBNP level of PAF group before and after card- ioversion had statistically significant difference( P 〈 0.05 ). ④The P wave dispersion of the control group and PAF group after cardioversion had statistically significant difference( P 〈 0.05 ). ⑤The NT-proBNP level and the P wave dispersion of PAF group after cardioversion showed significant positive correlation ( r = 0. 215 ). Conclusion Detecting the NT-proBNP level and P wave dispersion are of guiding significance for the diag- nosing or prognosis of PAF ,which is worthy of further research clinical application.
