胸腺蛋白口服液联合冰敷对鼻咽癌急性放射性口腔黏膜反应的临床疗效

Effect of thymus protein oral solution combined with ice compress on acute radiation-induced oral mucositis in treatment of nasopharyngeal carcinoma

摘要 目的探讨胸腺蛋白口服液(欣洛维)联合冰敷对鼻咽癌急性放射性VI腔黏膜反应的疗效。方法选择我院2010年3月至2012年9月行根治性放疗鼻咽癌患者75例随机分为观察组(n=38)与对照组(n=37),观察组给予胸腺蛋白口服液联合冰敷,对照组给予生理盐水漱口联合冰敷,两组用药均与放射治疗同期进行,观察比较两组急性放射性口腔黏膜反应发生时间及程度。结果观察组急性放射性口腔黏膜反应发生时间明显晚于对照组,两组比较差异有统计学意义(P〈0.05);放疗4周时,观察组与对照组I~Ⅱ级急性口腔黏膜反应发生率分别为68.42%、37.84%,llI~IV级分别为31.58%、62.16%,放疗结束时,观察组与对照组I~Ⅱ级急性口腔黏膜反应发生率分别为57.89%、16.22%,Ⅲ-IV级分别为42.11%、83.78%.两组反应严重程度比较差异有统计学意义(P〈0.05)。结论胸腺蛋白口服液联合冰敷可明显推迟和降低放射性口腔黏膜反应的发生,减轻口腔黏膜损伤。 Objective To investigate the effect of thymus protein oral solution combined with ice compress on acute radiation-induced oral mucositis in patients with asopharyngeal carcinoma. Methods Seventy- five patients with nasopharyngeal carcinoma underwent radical radiotherapy admitted to our hospital from Mar. 2010 to Sep. 2012 were randomly divided into observation group ( n = 38 ) and control group ( n = 37 ). The observation group was given thymus protein oral solution combined with ice compress and the control group was received gargling with normal saline combined with ice compress. The occurrence time and grade of acute radiation-induced oral mueositis were observed and compared between the two groups. Results The occurrence time of acute radiation-induced oral mucositis of the observation group was much later than the control group ( P 〈 0. 05 ). At 4 weeks after irradiation, the incidences of grade I - II acute radiation-induced oral mucositis of the observation group were 68.42% , 37.84% respectively, and grade Ⅲ - IV 31.58%, 62.16% respectively; the difference was statistically significant ( P 〈 0. 05 ). At the end of radiotherapy, the incidences of grade I - Ⅱ acute radiation-induced oral mucositis of the observation group were 57.89% ,16.22% respectively, and grade Ⅲ-Ⅳ 42.11% , 83.78% respectively; the difference was statistically significant (P 〈0. 05). Conclusions Thymus protein oral solution combined with ice compress can significantly delay and reduce the occurrence of radioactive oral mucositis, alleviate oral mucosal damage.
