BNP在机械通气撤机中的预测价值 被引量:6

摘要 目的:探讨自主呼吸试验(spontaneous breathing trials,SBT)前B型脑钠肽(B type natriuretic peptide,BNP)水平与机械通气患者撤机结局间的关系。方法:回顾性调查达到撤机标准并行撤机的66例患者,在患者达到撤机条件行SBT前抽血检测BNP水平,而后进行SBT,后予撤机拔管。根据48 h内的撤机结局,将患者分为成功组和失败组。比较两组间性别、年龄、APACHEⅡ评分、撤机前BNP水平及其对数值;绘制受试者工作特征曲线(receiver operating char-acterisitc curve,ROC曲线),评价撤机前BNP水平对撤机结局的预测价值。结果:66例患者中,12例(18.2%)撤机失败,失败病例中有4例通过了SBT但撤机失败。两组性别差异无统计学意义(P〉0.05),撤机成功组年龄相对较轻(P〈0.05)。撤机失败组SBT前BNP水平明显高于撤机成功组(P〈0.01)。 SBT前BNP预测撤机失败的ROC曲线下面积为(0.859±0.059),95%可信区间为(95%CI 0.751~0.932),其预测撤机失败的截点值为263 pg/mL,在此截点值下的敏感性为87%,特异性为75%。结论:撤机前BNP水平对撤机结局有一定的预测价值,可作为撤机筛查指标之一。
