体外冲击波碎石配合中西医结合治疗泌尿系结石的临床研究

Clinical study of extracorporeal shock wave lithotripsy combined with integrated traditional Chinese and Western medicine therapy in the treatment of urinary calculi

目的探讨泌尿系结石体外冲击波碎石后中药排石的临床疗效和1年后结石复发情况。方法筛选泌尿系结石直径≥10 mm的患者780例,随机分为治疗组和对照组,每组各390例,两组患者均先行体外冲击波碎石后分别接受中西医结合和纯西医治疗,40 d后观察两组患者的临床排石治愈率、疗程、1年后结石复发情况。结果治疗组肾结石、输尿管结石、膀胱结石治愈率均明显高于对照组,疗程缩短,复发率低(P<0.05)。结论泌尿系结石经体外冲击波碎石后再行中西医结合治疗,排石治愈率高,使很多具备手术指征的患者可以通过碎石非侵入性治疗而治愈。降低手术治疗率,降低医疗风险,减轻医疗费用及患者的痛苦,具有创伤小、病程短、康复快、复发率低等优点,是治疗泌尿系结石的理想选择。

Objective To explore the clinical curative effect and calculi recurrence 1 years later of urinary calculi after extracorporeal shock wave lithotripsy combined with Chinese medicine row of calculi. Methods 780 patients with uri- nary calculi and the calculi over 10 mm in diameter were randomly divided into treatment group and control group,each group of 390 cases.Both groups of patients were firstly treated with extracorporeal shock wave lithotripsy,then,the pa- tients in the treatment group were treated with integrated traditional Chinese and Western medicine therapy,and the control group with pure western medicine treatment.The clinical cure rate for calculi and the course of treatment and the relapse of eliminating calculi after 1 year were observed after 40 days. Results The cure rates of kidney calculi, ureteral calculi and bladder calculi in the treatment group was significantly higher than that of the control group re- spectively,with the shorter course of treatment and lower relapse rate （P〈0.05）. Conclusion After crushing calculi by adopting extracorporeal shock wave lithotripsy and then removing calculi by using integrated traditional Chinese and Western medicine therapy,the cure rate is high so that most patients with indication for surgery can be cured by adopt- ing noninvasive therapy in the treatment of calculi.With the characteristics of minimal invasive,short course of disease, fast rehabilitation and low relapse rate,this method can not only decrease the rate of surgical treatment and reduce medical risks,but also help to lighten medical costs and relieve pain of patients,which is the ideal choice for curing uri- nary calculi.
