妊娠晚期阴道菌群与妊娠结局的关系

Relationship between vaginal flora in late trimester of pregnancy and pregnancy outcome

目的探讨妊娠晚期阴道菌群与妊娠结局的关系。方法回顾性分析本院2010年5月～2013年5月收治的110例阴道菌群失调患者（试验组）及同期经实验室检查正常的100例妊娠晚期孕妇（对照组）的临床资料。结果在阴道感染常见致病菌中，以假丝酵母菌感染最为多见；观察组出现胎膜早破及产褥感染者分别占35．45％和28．18％，对照组出现胎膜早破及产褥感染者分别占10．00％和3．00％，两组间比较，差异均有统计学意义（P〈0．05）。观察组出现新生儿感染者占25．45％，明显高于对照组的1．00％（P〈0．05）。结论妊娠晚期阴道菌群感染常以假丝酵母菌感染为主，且对妊娠结局带来不同程度的影响，易出现胎膜早破、产褥感染及新生儿感染等，严重影响了母体及胎儿的存活质量，值得在早期进行诊断及预防。

Objective To discuss the relationship between vaginal flora in late trimester of pregnancy and pregnancy outcome. Methods Clinical data of 110 pregnant women with vaginal dysbaeteriosis in late trimester of pregnancy admitted into our hospital from May 2010 to May 2013 （experimental group） and another 100 pregnant women in third trimester were in normal conditions after laboratory examinations （control group） were analyzed retrospectively. Results As to common pathogenic bacteria in vaginal infeetion,Candida mycoderrna bacteria was the most common.In the experimental group,the incidences of premature rupture of fetal membranes and puerperal infection were 35.45% and 28.18% respectively,while in the control group,they were 10.00% and 3.00% in turn,compared between the two groups,the dif- ferences were all statistically significant （P〈0.05）.In addition,the incidence of neonatal infection in the experimental group was 25.45%,which was higher than that of the control group （1.00%）（P〈0.05）. Conclusion Canclida mycoderma bacteria as the most frequently seen bacteria in vaginal infection in late trimester of pregnancy,which has impacts on pregnancy outcome at varied degrees such as premature rupture of fetal membranes,puerperal infection and neonatal in- fection,ere.It seriously affected the qualities of maternal and fetal survival,which is necessary on early diagnosis and prevention.
