CPAP对小儿重症肺炎的疗效分析

Analysis on the efficacy of CPAP for treatment of children with severe pneumonia

目的：探讨小儿重症肺炎采用CPAP治疗的临床效果。方法本次共选择80例重症肺炎患儿做研究对象，均为我院2012年8月~2013年8月收治，采用数字表抽取法随机分组，就儿内科常规治疗方案（对照组，n=40）与加用CPAP治疗（观察组，n=40）临床效果进行比较。结果观察组选取病例经统计示临床总有效率为95%，显著高于对照组75%，差异有统计学意义（P＜0.05）。观察组气促、胸骨凹陷、发绀、肺部啰音缓解、平均住院天数均少于对照组，差异有统计学意义（P＜0.05）。两组疗前PaO2无明显差异，疗后PaO2均明显升高，观察组高于对照组升高幅度（P＜0.05）。结论小儿重症肺炎采用CPAP治疗，可显著改善预后，消除临床症状，加快患儿康复进程，具有非常积极的应用价值。

Objective To explore the clinical effects of CPAP for treatment of children with severe pneumonia. Methods 80 children with severe pneumonia admitted and treated in our hospital from Aug 2012 to Aug 2013 were selected as research objects.Based on the method of random number table, the 80 cases were randomLy assigned to the control group （routine pediatric treatment;n=40） and the observation group （routine pediatric treatment and CPAP treatment;n=40）.The clinical effects were compared between the two groups. Results Statistics indicated that the total effective rate in the observation groups was 95%,significantly higher than 75% in the control group, with statistical significance（P 〈 0.05）.The observation group had fewer cases of shortness of breath, sternum depression and cyanosis, greater alleviation of pulmonary rales,and shorter average hospital stay than the control group, with statistical significance （P 〈 0.05）.The two groups had no difference in PaO2 before treatment.PaO2 had significant increase in both groups after treatment,with greater increase in the observation group than the control group （P〈0.05）. Conclusion The application of CPAP on treatment of children with severe pneumonia can significantly improve prognosis, eliminate clinical symptoms and accelerate rehabilitation,with positive application values.
