摘要
目的评价持续肾脏替代治疗(CRRT)重症感染的有效性。方法计算机检索PubMed、万方、知网和维普等数据库1995年-2012年相关文献,进行质量评价和数据提取,使用RevMan 5.0软件进行系统分析。结果经筛选最终纳入5篇随机对照试验共计186例患者资料进行分析。Meta分析结果:CRRT能显著清除血IL-6[比值比(OR)38.88,95%可信区间(CI):7.72-70.04;Z=2.45,P=0.01]和TNF-α(OR 44.33,95%CI:13.80-74.86;Z=2.85,P<0.01),并能有效降低患者的病死率(OR 0.43,95%CI:0.22-0.84;Z=2.45,P=0.01)。结论 CRRT能显著清除患者体内的细胞因子,提高重症感染患者的生存率。
Objective To assess the effect of continuous renal replacement therapy(CRRT) on patients with severe infection. Methods Data related to the effectiveness of CRRT in the patients with severe infection were searched for in the databases of PubMed, Wanhng, CNKI and CQVIP published from 1995 to 2012, which were extracted and analyzed with Cochrane Collaboration' s RevMan 5 software. Results A total of 5 eligible studies(186 cases) was finally included. Compared with control group, Meta analysis showed that serum IL-6 (OR 38. 88, 95 % CI: 7.72-70. 04 ; Z: 2. 45, P: 0. 01) and TNF-a(OR 44. 33,95% CI: 13. 80-74. 86; Z: 2. 85, P〈0. 01) could be effectively removed by CRRT and the mortality was significantly lower in CRRT group(OR 0. 43,95% CI: 0. 22-0. 84; Z= 2.45,P:0. 01). Conclusion CRRT can effectively remove serum cytokines and increase survival rate of patients with severe infection.
