缺氧缺血性脑病新生儿血气和红细胞参数检测的意义 被引量:4

Significance of blood gas analysis and red blood cell parameters detection in newborns with hypoxic ischemic encephalopathy

摘要 目的探讨缺氧缺血性脑病(HIE)新生儿血气和红细胞参数检测的意义。方法检测并分析29例HIE患儿(A组)和30例正常新生儿(B组)血气和红细胞参数。结果 A组血气分析参数(pH、二氧化碳分压、氧分压、HCO-3、碱剩余)与B组均有统计学差异(P<0.01);A组红细胞参数检测结果(红细胞压积、平均红细胞容积、平均红细胞血红蛋白浓度、网织红细胞计数、未成熟网织红细胞比率、有核红细胞计数)与B组有统计学差异(P<0.05或P<0.01);两组红细胞、血红蛋白、平均红细胞血红蛋白含量等参数相仿(P>0.05)。结论监测HIE患儿血气可以及时指导纠正血液酸碱平衡紊乱,减少新生儿HIE后遗症的发生。监测红细胞参数,特别是观察未成熟红细胞数量,可以作为新生儿HIE辅助诊断的有效指标。 Objective To investigate the significance of blood gas analysis and detection of red blood cell(RBC) parameters in newborns with hypoxic ischemic encephalopathy(HIE). Methods The blood gas analysis and RBC-related parameters detection were performed and analyzed in 29 HIE newborns(group A) and 30 healthy babies(group B). Results The blood gas parameters of pH, PCO2 ,PO2 ,HCO3- and BE were worse in group A than those in group B(P〈0. 01). So did the red blood cell parameters of HCT, MCV, MCHC, RET, IRF, and NRBC (P〈0. 05 or P〈0. 01 ). However, there were no significant differences in the parameters of Rt, Hb, and MCH between two groups(P〈0. 05). Conclusion The blood gas monitoring in HIE newborns can guide the treatment of acid-base imbalance and reduce HIE-related sequelae. The red blood cell parameters, especially the erythrocytoblast may be taken as the indicators for auxiliary diagnosis and evaluation of HIE impairment in newborns.
