实时三平面超声心动图及全容积三维超声心动图评价酒精性心肌病患者左心房功能 被引量:6

Evaluation of left atrial function in patients with alcoholic cardiomyopathy with real-time tri-plane echocardiography and full-volume three-dimensional echocardiography

摘要 目的应用实时三平面超声心动图(RT-3PE)及全容积三维超声心动图(FV-3DE)评价酒精性心肌病(ACM)患者左心房功能。方法对31例ACM患者(ACM组)和年龄相匹配的30名健康正常人(对照组)行常规超声、RT-3PE及FV-3DE。常规超声及RT-3PE测量参数包括左心房最大容积(LAVmax)、左心房主动收缩前容积(LAVpre)、左心房最小容积(LAVmin)、左心房整体射血分数(LATEF)、左心房被动射血分数(LAPEF)、左心房主动射血分数(LAAEF);FV-3DE测量参数包括LAVmax、LAVmin、LATEF。对三种方法测得的LAVmax与LAVmin进行相关性分析。结果常规超声测得ACM组LAVmax、LAVpre、LAVmin、LATEF、LAAEF均高于对照组,LAPEF低于对照组(P均<0.05)。RT-3PE测得ACM组LAVmax、LAVpre、LAVmin、LATEF、LAAEF均高于对照组,LAPEF低于对照组(P均<0.05)。FV-3PE测得ACM组LAVmax、LAVmin、LATEF均高于对照组(P均<0.05)。三种方法测得的LAVmax与LAVmin具有良好相关性(P<均0.05)。结论 RT-3PE及FV-3DE能够准确测定ACM患者的左心房容积及功能。 Objective To evaluate left atrial function of alcoholic cardiomyopathy (ACM) patients with real-time tri-plane echocardiography (RT-3PE) and full-volume three-dimensional echocardiography (FV-3DE). Methods Totally 31 patients with ACM (ACM group) and 30 normal subjects (control group) underwent conventional echocardiography, RT-3PE and FV-3DE, respectively. Left atrial maximal volume (LAVmax), left atrial presystolic volume (LAVpre), left atrial mini- mal volume (LAVmin), left atrial total ejection fraction (LATEF), left atrial passive ejection fraction (LAPEF) and left atrial active ejection fraction (LAAEF) were measured with conventional echocardiography and RT-3PE, while LAVmax LAVmin and LATEF were measured with FV-3DE. The correlation of LAVmax and LAVmin were analyzed among three methods. Results In conventional echocardiography, LAVmax, LAVpre, LAVmin, LATEF and LAAEF in ACM group were higher, LAPEF in ACM group were lower than those in control group (all P(0. 05). In RT-3PE, LAVmax, LAVpre, LAVmin, LATEF and LAAEF in ACM group were higher, LAPEF in ACM group were lower than those in control group (all P(0.05). In FV-3DE, LAVmax, LAVmin and LATEF in ACM group were higher than those in con- trol group (all P(0.05). The correlation of LAVmax and LAVmin measured with three methods were all good (all P( 0.05). Conclusion RT-3PE and FV-3DE can assess left atrial volume and function accurately in patients with ACM.
