摘要
目的应用MR扩散加权成像(DWI)前瞻性研究其对脑实质囊虫病亚临床期的诊断价值。方法根据制定的纳入标准,筛选110例脑实质囊虫病(亚临床期)患者为研究对象,并与130例脑实质囊虫病活虫期对照,进行MR DWI检查,同时以表观扩散系数(ADC)值和指数表观扩散系数(eADC)值作为观测指标进行对照评价。结果脑实质囊虫病亚临床期组,正常对照镜像区组及活虫期组等各组间经统计学分析评价,差异有统计学意义,脑实质囊虫病亚临床期ADC值高于正常对照镜像区组,低于活虫期组;eADC值低于正常对照镜像区组,高于活虫期组,并应用受试者工作特征(ROC)曲线分析评价ADC值和eADC值对脑实质囊虫病亚临床期与活虫期鉴别诊断效能。结论揭示脑实质囊虫病亚临床期和活虫期病灶DWI水分子运动的特征,制定出相应的ADC值及eADC值诊断指标,并应用于亚临床期脑实质囊虫病的早期诊断,对于及早治疗具有非常重要的指导意义。
Objective To prospective study the value of MR diffusion weighted imaging in diagnosing subclinical neuro- cystieereosis. Methods According to established inclusion criteria, 110 cases of subclinical neuroeystieercosis were in- cluded as subclinical group and 130 cases of neuroeysticercosis with living worms were collected as living worm group. MRI diffusion weighted imaging was performed in all patients. ADC and eADC values were analyzed. Results There were sig- nificant difference of ADC and eADC among subclinical neurocystieercosis, normal control mirror area, and neuroeystic- ercosis with living worm. The ADC value of subclinical neurocysticereosis was higher than that of normal control mirror are- a, while lower than that of neurocysticereosis with living group. The eADC value of subclinical neuroeystieercosis was lower than that of normal control mirror area, while higher than that of neurocysticercosis with living worm. Conclusion ADC and eADC value can differentiate the cystieercosis of subclinical stage with living worms stage, which can apply to make early diagnosis and therapy of subclinical neuroeysticercosis.
