手动作观察训练对脑卒中失语症患者语言功能的影响 被引量:37

The influence of hand action observation training on language functions in a stroke patient with aphasia

摘要 目的:揭示手动作观察训练对失语症患者语言功能恢复的影响。方法:对1例经皮质运动性失语症患者进行为期3周的训练,要求患者观察操作物体的手动作视频(第1、3周),或者是观察不同物体的静态图片视频(第2周)。观察手动作视频或静态物体的同时对所观察物体进行复述。每次训练时间为30min,6次/周。每周训练前后进行60张国际标准图片命名检查和西方失语症成套测验(WAB)的失语商评定。结果:3周的训练对患者的国际标准图片命名、WAB失语商均有显著改善作用(P<0.05)。然而,手动作的观察与静态物体图片的观察相比,命名正确率和失语商的提高更为显著(P<0.05)。结论:手动作观察较静态图片观察训练更能改善该例经皮质运动性失语症患者的语言功能,该效应的机制以及此方法是否有助于其他类型失语症患者都值得进一步探讨。 Objective: To explore the effects of hand action observation training on the language function rehabilitation in patients with aphasia. Method: One patient with transcortical motor aphasia participants in this study. The whole training processes lasted for 3 weeks and the patient was asked to observe either videos containing hand movements relevant to the objects (the first and third week) or steady pictures of different objects (the second week), and to repeat the name of the objects observed. The training lasts 30 minutes a day and 6 days per week. The naming of 60 international standard pictures and aphasia quotient of western aphasia battery (WAB) were adopted before and after every week's training. Result: We found that the correct rate of picture naming and aphasia quotient increased significantly after ev- ery week's training (P〈0.05). In addition, the watching of hand action videos resulted in better rehabilitation in compared to the watching of steady-picture videos(P〈0.05). Conclusion: Hand action observation training improves the language function significantly better than steady pic- tures observation training for this aphasia patient. It needs further study to confirm the mechanism underlying this effect and the effects of this training on other types of aphasia patients.
