按揉结合治疗中风后肩关节半脱位的临床疗效分析

Analysis of Clinical Efficacy of Massage （Pressing and Kneading） for the Treatment of Post-stroke Glenohumeral Joint Subluxation

[目的]观察按揉二步推拿手法对于改善中风后肩关节半脱位（GHS）的作用。[方法]将100例中风后GHS患者随机分为观察组（50例）和对照组（50例），对照组采用良肢位摆放和康复训练，观察组在对照组的基础上增加按揉二步推拿手法，治疗前后分别给予Fugl-Meyer运动量表（上肢部分）评估和肩峰至肱骨头间距（acromio-humeral interval ，AHI）测量，比较两组间疗效的差异。[结果]经过6周治疗后，观察组上肢部分Fugl-Meyer评分显著高于对照组（P＜0．01），患侧AHI显著小于对照组（P＜0．05），观察组显效率优于对照组，且两组相比较差异有显著性（P＜0．05）。[结论]按揉二步推拿手法操作简单，治疗中风后GHS具有较好疗效，值得临床应用推广。

[Objective] To observe the effect of two-step massage（pressing and kneading ） on recovery of post-stroke glenohumeral joint subluxation（GHS） .[Methods]Totally 100 patients with GHS after stroke were randomly divided into observation group（ n=50） and control group（ n=50） .The control group received nor-mal limb positioning and rehabilitation training ,while the observation group was additionally given the two-step massage（pressing and kneading） based on the treatment of the control group .Fugl-Meyer exercise scale （upper limbs） for assessment was performed and acromio-humeral interval（AHI） were measured before and after the treatment .The efficacy was compared between two groups .[Results] After 6 weeks of treatment , Fugl-Meyer score for upper limbs in observation group was higher than that in control group （ P 〈 0 .01） , while AHI of the diseased sides was shorter than that in control group （ P〈0 .05） .The excellence rate of ob-servation group was superior to the control group （ P 〈0 .05） .[Conclusion]The two-step massage（pressing and kneading） is simple and effective for the treatment of post-stroke GHS .Therefore ,it is worthy of clinical promotion .
