退变性腰椎侧凸椎间盘真空现象的分布特点及影响因素 被引量:6

摘要 目的探讨退变性腰椎侧凸椎间盘真空现象的发生和分布特点及其相关影响因素。方法回顾分析2002-05-2012-05于我院治疗的78例退变性腰椎侧凸患者的影像学资料,以T12-S1范围内的椎间盘为研究对象,发生真空现象的椎间盘为真空组,未发生真快现象的椎间盘为非真空组,比较真空组与非真空组椎间隙变窄、椎间不对称、上位椎体旋转、侧方滑移、Modic改变的发生情况,分析侧弯范围内及侧弯范围以外椎间盘真空现象产生的影响因素。结果真空征组与侧凸组比较,椎间不对称、侧方滑移、上位椎体旋转的发生率均存在着统计学差异,两组椎间隙变窄及Modic改变的发生率无统计学差异。两组出现不同类型Modic改变的椎间隙个数比较,差异有统计学意义。结论退变性脊柱侧凸顶椎下位椎间盘真空现象的发生率最高,与椎体旋转及侧方移位有关;端椎下位椎间盘真空现象的发生与椎间不对称有关,侧弯范围以外椎间盘真空现象的发生与椎间隙变窄有关。 Objective To study the prevalence of intervertebral vacuum phenomenon in degener-ative lumbar scoliosis (DLS) and its influencing factors. Methods From May 2002 to May 2012, the imaging materials of 78 patients with DLS were retrospectively analyzed. Intervertebral discs between T12 and S1 were viewed,DLS with intravertebral vacuum phenomenon were selected as vac-uum group,DLS without intravertebral vacuum phenomenon were selected as non-vacuum group. The incidence of intervertebral narrowing,intervertebral asymmetry,superior vertebral rotation,lateral slippage and Modic changes were compared between the two groups. Results The incidence rates of intervertebral narrowing,intervertebral asymmetry,lateral slippage,superior vertebral rotation had significant differences between the two groups. No difference existed in intervertebral narrowing and Modic changes between two groups. The type of Modic changes had significant difference between the two groups. Conclusion Intervertebral vacuum phenomenon below apical vertebrae is mainly associated with vertebral rotation and lateral slippage,intervertebral vacuum phenomenon below end vertebrae is mainly associated with intervertebral asymmetry,and intervertebral vacuum phenomenon outside the scoliosis is mainly associated with intervertebral narrowing.
