脊髓血管母细胞瘤的MRI诊断与鉴别诊断

MRI diagnosis and differentiation of spinal cord hemangioblastomas

目的探讨脊髓血管母细胞瘤的MRI表现特征。方法回顾性分析8例经手术和病理证实的脊髓血管母细胞瘤的临床及MRI检查资料。结果 6例单发,2例多发,所有病灶均位于脊髓内,颈髓及胸髓各5个,肿瘤多呈点状或结节状、香肠状、葫芦形或不规则形,壁结节MRT1WI呈等信号,T2WI呈稍高信号,增强后肿瘤呈显著强化,肿瘤常继发囊肿或大范围脊髓空洞以及水肿,大于1cm的肿瘤内部及周围常见流空血管影。结论脊髓血管母细胞瘤的MRI影像表现具有特征性,掌握这些MRI表现有利于提高脊髓血管母细胞瘤的诊断正确性。

Objective To discuss the characteristic performance of MRI of spinal cord hemangioblastomas. Methods Clinical data and findings from MRI examination in 8 patients with pathologically confirmed spinal cord hemangioblastimas were retrospectively reviewed. Results The lesions were single in 6 cases and multiple in 2 cases. All lesions were located in the spinal cord, 5 case in cervical spinal cord, and 1 case in thoracic spinal cord. Tumors were point-like or nodular, sausage-like, gourd-shaped or irregular shape. The mural nodules were iso-intensity on T1 WI, hyper-intensity on T2 WI, the tumor was significantly enhanced after enhancement tumors were often mixed with cysts or a wide range of syringes, large （〉1 cm） tumors invariably had vascular flow voids or around the tumor. Conclusion The imaging performance of MRI in hemangioblastomas of the spinal cord is characteristic. It would be helpful for establishing the diagnosis preoper- ately while radiologists are getting aware of the features of spinal cord hemangioblastomas on MRI.
