Herbert螺钉治疗Ⅲ型尺骨冠突骨折伴肘关节脱位

Treatment of type III fractures of ulna coronoid process with elbow dislocation using Herbert screw

目的 探讨Herbert螺钉内固定对Ⅲ型尺骨冠突骨折伴肘关节脱位的治疗效果.方法 采用Herbert螺钉内固定治疗Ⅲ型尺骨冠突骨折伴肘关节脱位20例.结果 18例得到随访,随访时间7～28个月,平均16个月;骨折骨性愈合时间10～12周,平均11周;根据Morrey肘关节功能评定疗效：优12例（66.7%）,良5例（27.8%）,可1例（5.6%）,优良率为94.4%;均无深部感染、内固定松动或断裂、肘关节脱位及肘关节强直等并发症.结论 Herbert螺钉内固定治疗Ⅲ型尺骨冠突骨折伴肘关节脱位,手术创伤小、功能恢复好,是一种行之有效的手术方法.

Objective To evaluate the treatment effect of type m fracture of ulna coronoid process with elbow dislocation using the Herbert screw. Methods Twenty patients of type m fractures of ulna coronoid process with elbow dislocation were treated by internal fixation with the Herbert screw. Re- suits The following period of 18 patients ranged from 7 to 28 months,with an average of 16 months. The union time ranged from 10 to 12 weeks,with an average of 11 months. The clinical results were evaluated according to Morrey g scale. A total of 12 cases （66.7 % ） were rated as excellent, 5 （27.8 % ） as good and 1 （5.6%） as fair. The excellent and good rate was 94.4%. No deep infection,loosening or breakage of the internal fixators, dislocation of the elbow joint, stiffness of the elbow joint and other severe complications occurred. Conclusion In the treatment of typeⅢ fractures of ulna coronoid process with elbow disloca- tion ,internal fixation with Herbert screw has advantages of less trauma and better functional recovery, which is an effective method.
