急诊超声在腹部闭合性损伤中的临床价值

Efficacy of ultrasonography in diagnosis of closed abdominal injury in emergency practice

目的：分析75例腹部闭合性损伤患者的声像图特征，探讨急诊超声在腹部闭合性损伤中的临床价值。方法针对2009年10月~2012年12月中山大学附属第五医院收治的75例腹部闭合性损伤患者的声像图特征进行回顾性分析，并与手术结果进行比较。结果75例患者中，有67例经手术证实与超声检查结果相符合，准确率约89.33%；漏诊7例，为腹部单纯性损伤、联合性损伤及空腔脏器损伤；误诊1例为肝破裂；腹腔积液检出率100%。结论急诊超声在腹部闭合性损伤检查应用中具有便捷性、无创性、准确率高的特点，对临床治疗及随诊具有很重要的作用。

Objective To analyze the ultrasonographic characteristics of 75 patients with closed abdominal injuries and evaluate the values of ultrasonography in emergency practice. Methods Ultrasonographic characteristics of the closed abdominal injuries of 75 cases obtained from October 2009 to December 2012 were analyzed retrospectively, and compared with the results of surgical operation. Results Among the 75 cases, there were 67 cases which the ultrasonography were fully in line with their respective surgery, reflecting accurate rate of 89.33%, 7 cases of misdiagnosis including simple and multiple abdominal injuries and hollow organs injuries;1 case of liver rupture was misdiagnosis. All cases of ascites were detected by ultrasonography, 100%. Conclusions Ultrasonography in closed abdominal injuries is considered being fast, noninvasive, accurate and dynamic which is very important for emergency practice and the follow-up observation.
