运用共聚焦激光扫描显微镜在术前测定日光性角化病边界的效果分析 被引量:4

摘要 目的探讨运用共聚焦激光扫描显微镜术前检测日光性角化病边界的效果。方法应用共聚焦激光扫描显微镜观察日光性角化病患者皮损周边情况,确定肿瘤边界,与组织病理确定的边界进行分析对比,统计学分析应用t检验。结果应用共聚焦激光扫描显微镜对10例病理已确诊为日光性角化病患者皮损边缘处进行观测、记录,与病理检测边界进行对比,在统计学上无明显差异;将共聚焦激光扫描显微镜观测结果与常规手术切除边界进行对比,发现CLSM组明显低于常规值,具有统计学意义。结论共聚焦激光扫描显微镜可有效确定日光性角化病患者皮损边界,指导临床手术切除病灶。共聚焦激光扫描显微镜具有实时、无创的特点,在指导皮肤肿瘤切除方面具有广泛的应用前景,值得进一步推广。 Objective To define the lesional area of actinic keratoses by using confocal laser scanning microscopy (CLSM). Methods Our study utilized CLSM to observe the dermatic structure of actinic keratoses,identi- fied tumor boundaries compared to the histopathological images, aiming to find theoretical evidence in preop- erative detection of skin tumors with Confocal Laser Scanning Microscopy. Results 10 lesions of actinic ker- atoses were respectively analyzed by histopathological imaging and CLSM. No significant differences were ob- served between these two methods. The tumor margin suggested by CLSM was significantly smaller than the traditional surgical incision size. Conclusion CLSM can define tumor boundaries of the actinic keratoses patients effectively in vivo, which can be very helpful for surgical therapy. CLSM is a real-time and non-inva- sive detection method, having prospective applications in surgical resection of skin tumors.
