硬通道微侵袭技术治疗慢性硬膜下血肿

Treatment chronic subdural hematoma with hard channel micro-invasive technique

目的探讨硬通道微侵袭技术在慢性硬膜下血肿(CSDH)治疗中的应用。方法应用硬通道微侵袭技术、采用特制YL-Ⅰ型血肿穿刺针,治疗我院1991年～2000年收治的慢性硬膜下血肿181例。结果手术成功率100％;完全治愈177例,占97.8％;复发3例,占1.6％,再次应用硬通道微侵袭技术治愈;1例因肾功能衰竭死亡,占0.6％。结论硬通道微侵袭技术可在保持颅内压力基本稳定的情况下连续作业,并配合血肿腔内注入尿激酶清除固态血肿部分。此技术操作简单、安全,创伤小,费用低,疗效显著,易于掌握,治愈率高,不易复发,便于在各级医疗单位普及推广。

Objective To probe the hard channel micro - invasive technique for chronic subdural hematoma (CSDH) .Method Using special hematoma puncture needle type YL - I to treat 181 cases of CSDH admitted into our hospital from 1991 to 1998 with hard channel micro-invasive technique. Result The successful rate of operation was 100% .177 of 181 cases (97.8%) were cured.3 of 181 cases (1.6%) recurred with new hematoma after operation 1 of 181 cases (0.6%) died of renal failure after operation. Conclusion As the technique of hard channel micro -invasive less disturb intracranial pressure, it can be performed continuously and solid hematoma can be cleared out after injecting urokinase into the cavity of hematoma.The characters of this technique are: simple and safe to perform, less wound and expenses, notable curative effect, easy to learn, high cured and low recurred rate.It is easy to generalize and apply in all grades of hospitals.
