摘要
目的:调查2009年版国家基本药物在我市基层医疗卫生机构的临床应用情况,为2012年版国家基本药物目录的实施提供参考。方法:对全市公立基层医疗卫生机构实施基本药物制度前后的处方、门诊和住院费用、药品配备品种等情况进行调查研究。结果:基层医疗卫生机构的医生和护士能够掌握基本药物制度的相关内容,83.8%的患者通过各种渠道能够了解基本药物制度;实施基本药物制度后基层医疗卫生机构的药品价格下降约35%,门诊次均药品费用下降40.2%,单张处方金额下降20.8%,静脉给药率下降30%,抗菌药物使用率下降45%,激素使用率下降41.7%,同时存在基本药物品种短缺、部分药品采购不到、缺少全科医生等问题。结论:国家基本药物制度的实施减轻了居民的用药负担,应进一步加强国家基本药物制度的公众教育,2009年版基本药物目录存在一定的局限性,2012年版国家基本药物目录增加了药品数量,能够缓解群众的用药需求。
OBJECTIVE To investigate the implementation of 2009 version National Essential Drug system; to provide a ref- erence for the implementation of the 2012 edition of the National Essential Drugs list. METHODS The prescriptions, outpa- tient~ and hospitalized-patients" costs, varieties of drugs, etc before and after the implementation of National Essential Drug system were investigated in primary medical institutions. RESULTS Doctors and nurses in primary medical institutions could grasp the National Essential Drugs system, 83.8% of the patients through various channels were able to understand the essen- tial drug system; drug prices decreased by about 35% after the implementation of the essential drug system in primary medical institutions. The drug costs of clinic times decreased 40. 2%, single prescription decreased by 20.8%, intravenous rate de- creased by 30%, the usage of antimicrobial dropped 45%, hormone usage dropped 41.7%, while there was a shortage of es- sential drugs varieties, some drugs could not be procured, the lack of full GPs and other issues. CONCLUSION Implementa- tion of national essential drug system reduced the medication burden on residents, and it should further strengthen the public education of National Essential Drug system. The 2009 version of the National Essential Drug list had some limitations , the 2012 version of the National Essential Drug list increased the number of drugs that can relieve the massive medication require- ments.
