摘要
目的观察社区治疗轻型颅脑损伤病人的治疗效果。方法收集2010-08—11在我院神经外科门诊检查及初步治疗后的轻型颅脑损伤患者共512例,在社区健康服务中心按性别、年龄、受伤原因、头颅CT检查等项目分别登记、建立健康档案,行脑血循环剂、维生素及适当的心理治疗,分1周、15d、1个月、3个月进行随访与观察治疗效果。结果总治愈率99.0%;1个月内愈合率达到94.1%;男性女性治愈率相似;出现颅脑损伤后遗症共4例,其中3例为女性,1例61岁男性患者,头部撞伤后未行头颅CT检查,伤后22d出现呕吐,转医院检查诊断为慢性硬膜下血肿;年龄≤14岁组的总治愈率与15d内治愈率明显高于>55岁组;头皮损伤、颅骨骨折、颅内少量出血的患者治愈率差异无统计学意义(P>0.05)。结论对于在医院检查初步治疗后的轻型颅脑损伤患者,社区治疗也能达到很好效果,出现危象也能及时发现。
Objective To observe the efficacy of the community treatment on patients with mild craniocerebral injury. Methods Totally 512 cases with mild craniocerebral injury, who had neurosurgical examination and initial treatment in our hospital from Aug. 2010 to Nov. 2010, were admitted to our Community Health Service Center. And the patients were registered gender, age,cause of injury, image of cranial CT and other projects. Then they were given agents for improving, the cere- bral blood circulation vitamins and appropriate psychotherapy, and followed up at one week, two-week, one-month, three- month, and then the efficacy were observed. Results The overall cure rate was 99.0 % ; one-month healing rate was 94.1% ; the cure rates were similar between male and female; there were four patients having sequelae of traumatic brain injury: three cases were female, and another one was a 61-year-old male, who was injured by himself and had not made the CT examination, then vomited in the 22 days after injury, the patient was finally transferred to the hospital, and diagnosed as chronic subdural hematoma; the total cure rate and 15-day cure rate in the group of aged 414 years was higher than those in the group of aged 〉55 years; there were similar cure rates among the scalp injury, skull fracture, and intracranial bleeding in patients. Conclusion For the mild craniocerebral injury patients who have made the initial treatment in hospital, community-based treatment can achieve good results, and can discover the crisis timely.
