超声对胃肠穿孔的诊断价值分析

Diagnostic value analysis of ultrasound for gastrointestinal perforation

目的 探讨超声对胃肠穿孔的诊断价值.方法选择2012年1月～2013年6月在我院经超声检查且资料完整的胃肠穿孔患者96例,术前均行X线、超声检查.结果 70例患者超声显示出现腹腔游离气体,84例超声提示少到中量腹腔积液,96例患者中有84例X线检查发现膈下游离气体,占87.5%,90例超声提示为腹部包块 （胃穿孔20例,十二指肠穿孔62例,阑尾穿孔6例）.结论只有充分认识胃肠穿孔的超声特征,同时结合腹部X线检查及临床表现才能及时、准确地对胃肠穿孔疾病提供有效诊断.超声检查弥补了X线检查的不足,提高了胃肠穿孔的诊断率,为临床治疗方案,尤其手术方案的选择提供了可靠的指导依据.

Objective To evaluate the diagnostic value of ultrasound for gastrointestinal perforation. Methods Ninetysix cases gastrointestinal perforation patients with complete data,preoperative underwent uhrasonography and X-ray examination.Results Seventy patients ultrasound showed intraperitoneal gas appears, 84 cases ultrasound showed small to medium ascites volume. Among 96 cases, 84 cases X-ray showed free gas below the diaphragm,accounted 87.5%,90 cases ultrasound showed abdominal mass （gastric perforation 20 cases, duodenal perforation 62 cases ,perforation of the appendix 8 cases）. Conclusion Only fully understand the sonographic features of gastrointestinal perforation,combined with abdominal X-ray examination and clinical manifestations in order to provide effective diagnosis for gastrointestinal perforation disease.Uhrasonography for the lack of X-ray examination, improve diagnosis rate of gastrointestinal perfora- tion for clinical treatment programs, particularly provide a reliable basis for guiding for the choice of surgical solutions.
