DeBakey Ⅰ型夹层动脉瘤的外科治疗

Surgical treatment of aortic dissection (Debakey type Ⅰ).

目的 报告 5例 De Bakey 型夹层动脉瘤合并主动脉关闭不全的手术治疗结果。方法 切除升主动脉 ,“三明治”法加固主动脉断端 ,人工血管置换 ,3例同时行主动脉瓣成型术。结果 4例治愈出院 ,主动脉瓣返流消失或减轻 ,另1例手术死亡。结论 “De Bakey 型夹层动脉瘤应积极手术治疗。如合并主动脉瓣返流应争取行主动脉瓣成形术。

Objective To report the clinical results of aortic dissection (DeBakey type Ⅰ) associated with aortic incompetence in 5 patients treated at our hospital from January 1996 to June 2000.Methods Woven Dacron tubes were sutured into aorta proximally and distally after the ascending aorta was transected proximally and distally and each end was treated by an inner and outer felt bolster sewn into place with continuous sutures or interrupted mattress sutures.The aortic valve was repaired by Trusler's method of plication in 3 patients.Results One patient died during operation.The other four patients were well.The aortic incompetence of four patients disappeared or became mild.Conclusion [WT5”BZ]The treatment of aortic dissection (DeBakey type Ⅰ)should be surgically active.If the aortic dissection is associated with aortic incompetence and the aortic incompetencc is mild or moderate,the repair of aortic valve is rational.
