连续52例法乐四联症根治术无死亡报告

Total correction of Tetralogy of Fallot:a report of 52 consecutive cases without mortality.

目的 总结 5 2例 TOF根治手术的经验。方法 本治疗组共收治 5 2例 TOF病人 ,其中 8例手术前接受了右心造影检查。合并有肺动脉闭锁 3例 ;左肺动脉瓣膜闭锁 1例 ;左肺动脉起始处重度狭窄 2例。合并卵圆孔未闭或继发孔房缺 12例 ;左上腔静脉 6例。病人均接受了根治手术。 16例 (30 % )施行了跨瓣补片 ;1例主肺动脉闭锁用同种带瓣血管施行右心外管道连接。结果 本组病人均痊愈出院 ;术后随访 2～ 2 4月 ,除 2例有轻度右心衰 ,其余病人均恢复良好。结论 超声心动图检查能够满足大多数 TOF手术的需要 ;肺动脉狭窄的彻底解除是手术成功的关键。

Objective To review the clinical experience on total correction of Tetralogy of Fallot in 52 cases.Methods Our surgical team totally treated 52 patients with Tetralogy of Fallot.Only 8 cases received angiocardiography before operation.The associated cardiac anomalies included pulmonary atresia in 3 cases,patent foramen ovale or atrial septal defect in 12 cases,and left superior vena cava in 6 cases. All patients underwent total correction. 16(30%) cases needed transannular patch.1 pseudotruncus was repaired with a conduit of aortic homograft. Result There was no mortality in hospital. The patients have been followed up for 2～24 months.They were well except 2 who had mild right heart failure.[WT5”HZ]Conclusion Echocardiography can meet the need of most operative treatments for TOF.The key to the success of the operation is to relieve the stenosis of pulmonary artery completely.
