4例cisAB型献血者的结果分析

Analysis of 4 cases of cisAB blood group donors

目的对本站4例cisAB副标本血清学和基因分犁结果进行分析。方法RSP-100型自动加样仪检测本站2009年--2011年健康献血者的ABO血型，惟反定型不符或凝集明显较弱的血样均进行复核，并采用DNA序列分析技术进行基因型鉴定。结果4例献血者在ABO血型检测中均止反定型不符，表现为A强B弱，红细胞表面的H抗原比正常AB型明湿增强；反定型与AC均小凝集，与BC自＋～3＋的弱凝集，与0c均不凝集。血清学检测ABO血型可能为AB亚型，血清中有不规则抗B抗体。DNA序列分析结果发现，4例样本均包含cisABOl等位綦因（467C〉T，803G〉C），其基因型分别为cisABOI／0012例，cis ABOI／002和cisAB01／cisAB01各1例。结论舟山地区的cisAB血型的频率较其他地区高发，是否与舟山海岛环境相对封闭导致婚配问的血缘相近有关还有待进一步考证。

Objective To analyze the serology and genotyping results of 4 cisAB blood type samples from Zhoushan blood cen- ter. Methods RSP - 100 automatic instrument was used to detect ABO blood group of healthy blood donors from 2009 to 2011. The blood samples with inconsistent forwaM and reverse blood typing or significant weak agglutination were reviewed, then direct sequencing was performed to characterize genotype of the 4 discrepant samples. Results The ABO blood groups of the 4 blood donors were all inconsistent in forward and reverse blood typing test, showing strong A and weak B. H antigen was stronger than normal AB group. The plasma exhibited no agglutination with A cells or C cells, but ＋ - 3 ＋ weak agglutination with B cells. The four samples were identified as AB subtype by serological technology and anti - B irregular antibodies were found. Four samples all included cisAB01 allele genotypes, 2 cisABOl/O01 cases, 1 cisABO1/O02 case and 1 cisABO1/ cisABO1 case by direct DNA sequencing. Conclusion The cisAB type is more frecquent in Zhoushan area than other regions. Whether the closed environment in Zhoushan islands leading to the similar blood relationships need further research.
