早期胃癌淋巴结转移影响因素分析及术式选择

Analysis of Factors For Lymph Node Metastasis in Early Gastric Cancer and Selection of Operating Method

目的:探讨早期胃癌(early gastric cancer,EGC)淋巴结转移的影响因素,为术式选择提供参考依据。方法:回顾性分析我院343例早期胃癌患者的临床资料,分析其淋巴结转移情况与各相关因素之间的关系及术式选择。结果:343例早期胃癌患者中,行根治性远端胃切除术(D2)206例;根治性近端胃切除术(D2)58例;根治性全胃切除术79例(D2)。其中51例发生了淋巴结转移,转移率为14.87%。单因素分析结果显示,淋巴结转移与肿瘤的大小、分化程度、浸润程度等密切相关(P<0.05);与年龄、性别、大体分型、肿瘤部位、病理学类型等无明显关系(P>0.05)。结论:淋巴结转移与肿瘤的大小、分化程度、浸润程度等因素相关,这些因素对于术前判断淋巴结转移、选择合理的手术方式等提供重要的依据。

Objective：To investigate the factors for lymph node metastasis in early gastric cancer and provide the basis for selection of operating method. Methods： The clinical data in patients with early gastric cancer patients（ n = 343） in our hospital were analyzed retrospectively. The analysis was performed to investigate the correlation between the lymph node metastasis and related factors and select the operating method. Results： Radical distal gastrectomy（D2） was used for 206 patients , radical proximal gastrectomy（D2）was used for 58 patients and radical total gastrectomy（D2）was used for 79 patients in 343 EGC patients. 51 cases were found the lymph node metastasis （14.87 % ） in 343 EGC patients. The results of univariate analysis showed that lymph node metastasis had a close relationship with tumor size, infiltration degree and differentiation degree （P 〈 0. 05）, while no statistical relationship was showed for gender, age and tumor location（ P 〉 0.05）. Conclusion：The lymph node metastasis had a close relationship with tumor size, infiltration degree and differentiation degree , these factors will provide the important basis for the pre - operating evaluation of lymph node metastasis and the reasonable selection of operating method.
