摘要 目的:研究人参提取物对兔耳增生性瘢痕的作用,探讨其作用的相关分子的表达.方法:建立15只大耳白兔的增生性瘢痕模型,每只兔耳腹侧中段随机做2~3处伤口,并分为空白对照组a,生理盐水注射组b,人参皂苷Rb1 c组(0.2 mg/ml),d组(0.4 mg/ml),e组(1 mg/ml),5组.术后任其愈合,观察创面愈合和瘢痕增生情况.上皮化后26 d按组别进行相应处理,每3d给药1次,共3次,用药后分别于第2,4,6周取材,每次取5只兔子,进行HE染色,Masson染色,测量并计算HI,计算胶原纤维密度,免疫组织化学测Caspase 3的表达.结果:a,b组间差异无统计学意义(P>0.05),a,b组分别与c,d,e组间的差异有统计学意义.随药物浓度增加HI、胶原密度下降,但c,d组间差异无统计学意义(P>0.05),e组与c组和d组间差异有统计学意义(P<0.05).同样见于Caspase 3的组织化学结果,高浓度干预组Caspase-3表达增加.结论:人参提取物Rb1成分能减少瘢痕组织中胶原的合成,诱发细胞凋亡的基因表达,从而有效抑制增生性瘢痕的发展. Objective. To study the effect of ginseng extract on Caspase-3 in rabbit ear so as to investigate the prospect of hypertrophic scar treatment. Methods: 15 hypertrophic scar models were established, and 2 to 3 wounds in the ventral and middle ear of each rabbit were done randomly. Then they were divided into the blank control group ( Group A), the saline injection group (Group B), and the ginsenoside Rbl groups (Group C,D and E). Group C, D, E were injected with 0.2 mg/ml, 0.4 mg/ml and 1 mg/ml ginsenoside Rbl. The wound healing and scar hyperplasia after operations were generally observed. Epithelization was conducted the corresponding treatment by group after about 26 days by being administered once every three days with a total of 3 times. After hay- ing been administered, scar tissue specimens of the rabbits were taken on the 2nd , 4th and 6th week respectively, 4 rabbits each time to have HE staining and Masson staining. Then HI and collagen fiber density were measured and calculated, and the expression of Caspase 3 was detected by immunohistochemical measurement. Results: With the increase of drug concentrations of HI, collagen density decreased, but there was no difference between Group C and Group D (P〉0. 05), and three were differences between Group E and Group C and D (P〈0.05). Histochemical results were also found in Caspase 3 that Caspase-3 expression increased with the intervention of high concentration of drugs. Conclusion. Rbl components of ginseng extract can reduce the synthesis of collagen in the scar tissue and induce the cell apoptosis, thus effectively inhibit hypertrophic scar.
