机械通气辅助治疗急性左心衰竭的临床应用

Clinical applicafion of mechamial ventilation in treatment of aute left ventricmlar failure

目的 :探索机械通气辅助治疗急性左心衰竭的临床应用效果。方法 :2 6例急性左心衰竭患者在给予常规药物治疗的基础上 ,根据临床情况辅以机械通气治疗 ,其中 16例采用双相气道内正压呼吸机 (Bi PAP) ;10例采用紧急气管插管接纽帮 10 0 Ei型呼吸机。结果 :机械通气治疗 2 h后 ,2 6例动脉血气分析中的 Pa O2 、Pa CO2 较通气前明显改善 (P<0 .0 5 ,P<0 .0 5 ) ,其中 2 0例达到治疗效果并顺利撤机。有 6例因原发病严重而死亡。结论 :根据临床情况迅速合理应用机械通气治疗 ,能及时有效缓解急性左心衰竭的低氧血症、高碳酸血症 ,改善呼吸功能 ,有利于心力衰竭改善 ,是抢救急性左心衰竭的有效措施。

Objective:To explore the effects of mechanical ventilation the treatment of acute left ventricular failure.Methods:While the routine drug therapy were abministered for 26 patients with acute left ventricular failure,16 patients received BiPAP,and 10 patients received mechanical ventilator followed urgent tracheal intubation.Results:PaO 2andPaCO 2were dramatically improve two hour after the mechanical ventilation were used( P <0.05).20 patients had a good results,and 6 patients died.Conclusion:Based on the clinical condittion,the mechanical ventilation could improve effectively hypoxia,hypercarbia and respiratory function caused by acute left ventricular failure,which is and effective method in the treatment of patient wth acute left vertricular failure.
