摘要
目的通过观察不同合金烤瓷冠修复前后龈沟液(GCF)中可溶性细胞间黏附分子-1(sICAM-1)及白细胞介素-1β(IL-1β)水平变化,探讨不同金属基底烤瓷冠对牙周组织的影响。方法将45颗牙分为镍铬合金组、钴铬合金组和金铂合金组,每组15颗,分别进行镍铬合金、钴铬合金和金铂合金PFM全冠修复。在治疗前及治疗后6个月检测受试牙菌斑指数(PLI)、龈沟出血指数(SBI)、龈沟探诊深度(GCD)、龈沟内GCF量和sICAM-l、IL-1β水平。结果镍铬合金组修复后6个月,SBI、GCD和GCF量和sICAM-l、IL-1β水平[(3.72±0.33)mm、(1.62±0.24)mm、(0.691±0.112)mg、(94.12±25.14)ng/mL、(8.49±3.22)ng/mL]与钴铬合金组[(3.03±0.35)mm、(1.18±0.18)mm、(0.419±0.131)mg、(74.20±26.34)ng/mL、(5.72±3.06)ng/mL]、金铂合金组[(2.21±0.27)mm、(1.42±0.21)mm、(0.325±0.115)mg、(64.89±24.01)ng/mL、(4.24±2.18)ng/mL]相比,差异均有统计学意义(均P<0.05)。且钴铬合金组修复后6个月,SBI、GCD和GCF量和sICAM-l、IL-1β水平与金铂合金组相比,差异有统计学意义(P<0.05)。结论金铂合金烤瓷全冠对牙周组织的不利影响小于镍铬合金、钴铬合金,钴铬合金优于镍铬合金。
Objective To investigate the effect of different alloy porcelain crowns on periodontal tissue by means of measuring the level of soluble intercellular adhesion molecule-1(sICAM-1) and interleukin-1β(IL-1β) in gingival crevicular fluid(GCF) after treatment. Methods A total of 45 teeth were divided into three groups(Ni-Cr alloy group,Co-Cr alloy group and Au-Pt alloy group, 15 teeth of each group), and treated by Ni-Cralloy, Co-Cr alloy and Au-Pt alloy PFM crown according grouping. At the point of pre-restoration, 6 months after cementation, the plaque index(PLI), gingival sulcus bleeding index(SBI) and gingival crevice depth(GCD), GCF and the level of sICAM-1 and IL-1β were detected. Results At the point of 6 months after treatment, SBI, GCD and GCF and the level of sICAM-1 and IL-1β of Ni-Cr alloy group [(3.72±0.33)mm,(1.62±0.24)mm,(0.691±0.112)mg,(94.12±25.14)ng/mL,(8.49±3.22)ng/mL] compared with those in Co-Cr alloy group [(3.03±0.35)mm,(1.18±0.18)mm,(0.419±0.131)mg,(74.20±26.34)ng/mL,(5.72±3.06)ng/mL] and Au-Pt alloy group [(2.21±0.27)mm,(1.42±0.21)mm,(0.325±0.115)mg,(64.89±24.01)ng/mL,(4.24±2.18)ng/mL], there were significant differences(all P 0.05). At the point of 6 months after cementation, CoCr alloy group showed significant difference for SBI, GCD, GCF and the level of sICAM-1 and IL-1β showed significant difference when compared to Au-Pt alloy group(P 0.05). Conclusion Au-Pt alloy has less bad effect on the periodontal tissue than Ni-Cr alloy and Co-Cr alloy, while Co-Cr alloy has better effect than Ni-Cr alloy.
