骨巨细胞瘤的治疗策略

Therapeutic strategy of giant cell tumor

目的探讨骨巨细胞瘤的临床治疗策略。方法选择2008年1月～2011年12月本院收治的27例骨巨细胞瘤患者作为研究对象，根据Campanaeci分级选择治疗方案，17例患者（A组）采取扩大刮除联合骨水泥填充治疗．10例患者（B组）采取手术切除后肿瘤关节置换，术后随访24个月，评价临床效果。结果A组患者肿瘤复发率为17．65％，B组未见复发病例，两组间比较，差异有统计学意义（P〈0．05）；A组的MSTS肢体功能评分为（20．8±3．6）分，B组的评分（24．8±3．8）分，组间比较差异有统计学意义（P〈0．05）。结论扩大刮除骨水泥填充术后的复发率相对较高．手术切除后肿瘤关节置换的肿瘤清除效果显著，膝关节功能恢复良好，值得推广应用。

Objective To investigate？therapeutic strategy of giant cell tumor. Methods 27 cases of patients with giant cell tumor were selected as research object,who stayed in our hospital from January 2008 to December 2011.According Campanacci classification,expand curettage joint with bone cement filling was taken by 17 cases patients （group A）,and tumor resection combiend with arthroplasty was taken by 10 cases （group B）.Clinical effects were then evaluated by follow-up for 24 months. Results The tumor recurrence rate of group A was 17.65%,and group B had no recurrence. The difference between groups was statistically significant（P〈0.05）.MSTS limb function score of group A was（20.8±3.6） points,and group B was（24.8±3.8）points.The difference between groups was statistically significant（P〈0.05）. Conclusion Postoperative recurrence rate of expand curettage joint with bone cement filling is relatively higher.Efficacy of removing tumor about tumor resection combiend with arthroplasty is significant,with a good knee function recovered,which is worthy of application.
