摘要
目的用彩色多普勒超声(CDUS)监测糖尿病肾病患者左肾动脉(LRA)血流动力学改变。方法应用聚合酶链反应-限制性片段长度多态性技术对156例糖尿病肾病患者的CD14基因启动子-159位点进行基因型分析,将他们分为CC基因型和CT+TT基因型2组,应用CDUS测量其LRA大血流速度(Vmax)、舒张小血流速度(Vmin)、平均血流速度(Vmean)、搏动指数(PI)、阻力指数(RI)等血流动力学有关参数。结果 CC基因型糖尿病肾病患者LRA Vmax、Vmin、Vmean血流速度积分(VTI)减低、Pl、RI增加,与CT+TT基因型糖尿病肾病患者比较存在显著性差异(P<0.05,P<0.001)。结论 CD14启动子-159(C-T)基因多态性与糖尿病肾病患者肾脏血流动力学改变存在相关性。CDUS通过监测糖尿病肾病患者肾脏血流动力学改变,为其诊断和预后评估提供了种无创监测的方法。
[ Objective ] To evaluate the application of Doppler ultrasound to monitor hemodynamic changes of left renal artery (LRA) in diabetic nephropathy (DN) patients. [Methods ] Genotypes of CD14 were typed in 156 DN pa- tients by polymerase chain reaction-restriction fragment length polymorphism. Then the patients were divided in two groups, CC genotype and CT+TF genotype. The maximal systolic velocity (V-max), the minimal diastolic velocity (V-mean), the mean velocity (V-mean), the time velocity integral (TVI), the resistance index (RI) and the pulsatility index (PI) of the LRA were measured with Doppler ultrasound. [Results] Vmax, V-mio, V-mean and TVI were decreased but RI and PI increased in CC genotype, and there is significant difference with CT+TF genotype (P 〈0.05, P 〈0.001). [ Conclu- sions ] The distribution of CD 14 promoter gene-159 (C-T) polymorphism has relation with hemodynamic changes of left renal artery (LRA) in DN patients. We can utilize Doppler ultrasound to non-invasively monitor the hemodynam- ic changes of LRA in DN patients.
