HLA-B27与强直性脊柱炎的相关性研究

Study on the correlation of HLA-B27 and ankylosing spondylitis

目的探讨应用流式细胞术检测人类白细胞抗原B27（HLA-B27）在临床上对强直性脊柱炎（AS）的诊断价值。方法应用流式细胞术,选择来本院就诊的门诊、住院的AS患者（AS组）561例及健康体检者（健康对照组）1 000例,进行HLA-B27检测。结果两组间HLA-B27阳性率比较：AS组为88.95%,健康对照组为6.90%,两组比较差异有显著性（P〈0.01）。两组不同性别间HLA-B27检测阳性率比较：临床确诊的AS组561名患者中,HLA-B27表达阳性患者499名（88.95%）,其中男性364名（64.88%）,女性135名（24.07%）,AS组男女比例约为2.7∶1,阳性率差异有显著性（P〈0.01）;健康对照组1 000例中HLA-B27表达阳性患者69名（6.9%）,其中男性39名（3.9%）,女性30名（3.0%）,健康对照组男女比例约为1.3∶1,阳性率差异无显著性（P〉0.05）。两组不同年龄段间HLA-B27检测阳性率比较：临床确诊的AS组561名患者,HLA-B27表达阳性患者中21~40年龄段298名（52.46%）,21~40年龄段阳性率差异有显著性（P〈0.01）,其他年龄段的阳性率无显著性差异（P〉0.05）;健康体检组HLA-B27表达阳性患者各年龄段的阳性率无显著性差异（P〉0.05）。结论 HLA-B27与AS具有高度相关性,检测HLA-B27有助于早期发现AS患者,以实现早期诊断,早期治疗。

[ Objective ] To discuss the application flow cytometry（FCM） detection HLA B27（HLA-B27） in clinical of ankylosing spondylitis （AS） diagnostic value. [Methods] HLA-B27 detection was conducted by flow cytometry （FCM） in 561 patients of AS （AS group）, and 1000 healthy individuals. [ Results ] Between the two groups HLA-B27 positive rate comparison： AS group is 88.95%, the healthy individuals group is 6.90%, two groups of comparisons differences are significant （P 〈0.01）. In clinically diagnosed of 561patients with AS, HLA-B27expression positive patients with 499 （88.95%）, of which 364 male patients （64.88%）, the female 135 （24.07%）, the positive ratio was statistical difference in males to females with HLA-B27 positive expression was about 2.7：1 （P 〈0.01）. 1000 healthy individuals controls, HLA-B27expression positive patients with 69（6.9%）, the male 39 （3.9%）, the female 30 patients （3.0%）, the positive ratio was no statistical difference in males to females with HLA-B27 positive expression was about 1.3：1 （P 〉0.05）. Two groups between different age paragraph HLA-B27 testing positive rate comparison： In diniea[ty diagnosed of 561patients with AS, HLA-B27 expression positive patierits , 21-40 ages 298 （52.46%）, the positive ratio was statistical difference in 21-40 age （P 〈0.01）, and the other positive rate of age no statistieal significance （P 〉0.05）. The healthy individuals group FILA-B27 expression positive patients of different age stages in the positive rate of no statistical significance （P 〉0.05）. [ Conclusions ] The detection of HLA-B27 plays important roles in the early diagnosis and early treatment of patients with AS.
