摘要
医师多点执业的根本意义在于通过市场对资源的配置作用,促进医务人员的合理流动,进而实现合理配置卫生人力资源、促进多元化办医、体现医务人员劳务价值、促进公立医院改革等目标。本文从制度环境、主要参与者利益及行为、医师多点执业的政策制约因素等方面进行分析,认为制度环境和以医院为主的参与者利益取向及其行为对医师多点执业具有更重要的影响。最后,结合国际经验,提出改革医院事业单位体制、实现医师从单位人向社会人转变、医院与医生的人事关系转为契约雇佣关系,以及改变政府治理方式、放权于社会等建议。
The fundamental significance of multiple-site physician practice policy lies in promoting the flow of medical personnel in order to rationally allocate health human resources, diversify the medical environment, reflect the real value of medical staff, and force change in public hospitals. The paper analyses restriction factors of multiplesite physician practice policy from institutional environment, interests and behaviors of main actors, and policy content. The first two may play a more important role in this policy. Finally, Based on international experience, this paper recommend reforming the hospital institution system, changing the relationship between doctor and hospital from hierarchical to employment-based, and changing the governance mode.
