Orem护理模式对康复期分裂症患者社会功能和自尊及总体幸福感的影响

The Effect of Clinical Orem Mode on Maintaining Self-respect and Overall Well-being in Patients with the Recovery Schizophrenia

目的:探讨Orem护理模式对康复期精神分裂症患者社会功能、自尊、总体幸福感的影响。方法:随机选取2008-2012年本院住院治疗的90例精神分裂症康复期患者的完整资料,根据护理方式的不同,将患者分为观察组与对照组,对照组接受精神科常规治疗和护理,观察组在常规治疗和护理的基础上,另接受以Orem护理理论为框架的特别护理干预。干预前及护理干预后的12周分别以自尊量表(SES)、住院患者观察量表(NOSIE-30)、总体幸福感量表(GWB)对两组患者护理干预前后的状态进行评估。结果:治疗12周后,两组精神状态的恢复情况差异不明显(P>0.05);观察组在采用Orem的自理模式12周后,社会能力、个人卫生、社会兴趣的积极因子方面显著提高,激惹、退缩、抑郁的消极因子改善明显,与对照组比较差异有统计学意义(P<0.05);观察组在GWB评分与SES评分方面,与对照组比较差异有统计学意义(P<0.01)。结论:运用Orem护理模式对改善康复期精神分裂症患者的社会功能、自尊状态及总体幸福感方面均有显著的效应,具有显著的临床推广价值。

Objective： To investigate the effect of clinical Orem mode on maintaining the serf-respect and overall well-being in patients with recovery schizophrenia. Method： Clinical data of 90 patients with schizophrenia in period of convalescence were investigated. According to the different methods of nursing care, the patients were randomly divided into observation group and control group. The control group received routine treatment and psychiatric nursing, on the basis of routine treatment and nursing, the observation group received Orem seLf-care mode. The samples were take out for assessment after 12 weeks by SES, NOSIE-30, GWB. Result： After treatment for 12 weeks, mental state recovery of two groups was not obvious difference （ P〉O.05 ） . Through Orem serf-care mode intervention, the self-esteem, seff-efficacy and social support could be improved, irritability, depression and other negative emotions were improved, compared with the control group, the differences were significant （ P〈0.05 ） . The GWB score and SES score of the observation group were higher than the control group, compared with the control group, the differences were significant （ P〈0.01 ） . Conclusion： Orem seLf-care mode has remarkable effect of improving the state of social function, self-esteem and genera] weE-being of patients with schizophrenia, has significant clinical value.
