Nursing Care of Glaucoma Patients Undergoing the Trabeculectomy Combined with Glaucoma Drainage Implants

目的:总结小梁切除青光眼引流器植入术治疗青光眼的护理经验。方法:回顾性分析2012年1-11月本科室收治的93例(135眼)行小梁切除青光眼引流器植入术治疗青光眼患者的临床资料,并总结围手术期的护理要点。结果:93例(135眼)术程顺利,经耐心周到的护理和术后准确、及时的抗炎、止血等治疗后,均康复出院。所有患者均未出现心理应激反应,22眼出现术后并发症,其中11眼浅前房伴低眼压,10眼暂时性高眼压,1眼出现脉络膜脱离,经及时发现积极治疗护理,全部得到有效控制。术后随访3个月,眼压降至(15.94±6.05)mm Hg(1 mm Hg=0.133 kPa),与术前眼压(48.12±7.29)mm Hg比较差异有统计学意义(t=16.733,P<0.05),视力无进一步下降,视野无进一步缩小。93例完全成功率为71.1%(96/135),部分成功率为12.6%(17/135),总成功率为83.7%(113/135)。结论:术前的心理护理和术后并发症护理是小梁切除青光眼引流器植入术治疗青光眼成功的重要保证。

Objective：To summarize the nursing experience of glaucoma patients undergoing the trabeculectomy combined with glaucoma drainage implants. Method：93 patients（135 eyes）were analyzed retrospectively in our hospital from January 2012 to November 2012,which were treated by the trabeculectomy combined with glaucoma drainage implants,and summarized the nursing points during perioperative period. Result：93 cases（135 eyes）routine operation smoothly,the patience and considerate care and postoperative accurate,timely after the treatment,such as anti-inflammatory, hemostatic,rehabilitation hospital discharge. All patients did not appear the psychological stress reaction,22 eyes had postoperative complications, including 11 eyes had shallow anterior chamber with low intraocular pressure（IOP）,temporary high intraocular pressure in 10 eyes,1 eyes appeared choroidal detachment,all had been effectively controlled during the timely detection and active treatment and nursing care. Follow up of 3 months, intraocular pressure decreased to（15.94±6.05）mm Hg（1 mm Hg=0.133 kPa）,compared with pre-operation（48.12±7.29）mm Hg,IOP was significant （t=16.733,P〈0.05）. Vision without further down and visual field no further narrow. 93 cases with 135 eyes completely success rate was 71.1%（96/135）, part of the success rate was 12.6%（17/135）,the overall success rate was 83.7%（113/135）. Conclusion：The nursing of Preoperative psychological and postoperative complications is an important guarantee for the success of the trabeculectomy combined with glaucoma drainage implants to treat glaucoma.
