彭氏胰胃吻合术在胰十二指肠切除术应用32例分析

Duodenopancreatectomy using “Peng ” pancreas-gastric anastomosis

目的探讨彭氏胰胃吻合术在胰十二指肠切除术中的应用效果。方法回顾性分析2010年4月至2013年5月间接受胰十二指肠切除术后胰胃吻合术32例患者的临床资料。结果 32例患者手术均顺利完成,平均手术时间(366.2±71.8)min,术中平均输血量(420.2±100.4)ml;术后平均住院时间(21.3±10.7)d;术后并发症总发生率25.0%(8/32),其中淋巴瘘1例(3.1%),胃排空障碍1例(3.1%),切口感染2例(6.3%),腹腔积液2例(6.3%),肺部感染2例(6.3%),均经保守治疗后痊愈,无住院期间因手术死亡病例。结论彭氏胰胃吻合术是一种安全、可靠、有效的胰腺残端重建术式。

Objective To investigate the clinical outcome of ＂Peng＂ pancreas-gastric anastomosis in duodenopancreatectomy. Methods A retrospective analysis of was performed in 32 patients who had received duodenopancreatectomy by using ＇＂ Peng＂ pancreas-gastric anastomosis from April 2010 to May 2013. Results The 32 patients suceessed in operation with and average operative duration of （366.2 ＋ 71.8） minutes, ~ivem^e~/blood transfusion of （420. 2 ~ 100. 4） ml, average hospital stay of （21.3_＋ 10.7） days, and a complication rate of 25.0% （8/32） including lymph leakage in 1 patient （3.1%）, dysfunction of gastric emptying in 1 （3.1% ）, incision infection in 2（6.3% ）, seroperitoneum in 2（6.3% ）, and pulmanary infection in 2 （6.3 % ）. All of the complications were cured by conservative treatment, with no perioperative death. Coneluslon ＂Peng＂ pancreas-gastric anastomosis is one of the safe, reliable and effective reconstruction patterns in duodenopanereatectomy.
