圆锥角膜患者角膜特性改变对视盘和视网膜神经纤维层的影响

摘要 目的观察圆锥角膜患者角膜特性改变对视盘和视网膜神经纤维层的影响。方法圆锥角膜患者作为观察组(68例,68眼),健康受试者作为对照组(85例,85眼),每位受试者均经电脑验光仪、裂隙灯显微镜、眼底、前房角镜、视野、角膜地形图、中央角膜厚度及眼轴检查。结果最终纳入统计分析的观察组受试者46例(46眼),对照组受试者74例(74眼)。两组年龄、性别比及等效球镜差异无统计学意义(P>0.05),最佳矫正视力、中央角膜曲率、中央角膜厚度、眼轴及眼压差异均有统计学意义(均P<0.05)。观察组视盘面积、视杯面积、杯盘面积比及视杯深度均比对照组大(均P<0.05),两者的线性C/D两组差异无统计学意义(P>0.05)。进一步研究发现,RNFL厚度发现仅鼻下方变薄外(P<0.05)。结论圆锥角膜患者的视盘有更大的面积,以及更大更深的视杯,此差异导致圆锥角膜患者的视功能更易受眼压的影响。
