脑桥海绵状血管瘤手术时机的把握及手术入路的选择 被引量:2

摘要 海绵状血管瘤又称海绵状血管畸形,其血管壁由单层内皮细胞组成,缺乏肌层和弹力层.脑干海绵状血管瘤占颅内海绵状血管瘤的9%~35%,年出血率0.7%~6.0%,是幕上病变的30倍,且多数发生在脑桥[1,2].部分患者无明显症状,仅在体检或颅脑损伤做头颅CT检查时偶然发现,一部分患者可有轻微或明显的神经系统受累表现,如颅神经麻痹、意识障碍、共济运动障碍和长束征等.海绵状血管瘤的自然史包括静止、增大、出血和自发缩小.脑桥海绵状血管瘤的治疗主要有随访观察、手术治疗和放疗等方法,其中对于无症状或症状轻微偶然发现的多采用随访观察的手段,而手术干预是针对神经功能进行性加重或有反复出血倾向者,手术的目的是全切肿瘤、解除占位效应、减少出血的发生和最大限度地保留神经功能.有关放疗的疗效尚待进一步探讨,因为目前尚无大宗远期随访观察的前瞻性病例对照研究.
