延续护理在托老院压疮高危人群中的应用及成效

The Application and Effectiveness of Continuing Nursing Care among Population at High-risk for Pressure Sore in Aged Care Facility

目的探讨延续护理在托老院高危人群中预防与治疗压疮的应用价值。方法将本市2011年4月-2013年4月28家托老院中的282人纳入研究范围,予以延续护理,对护理员进行严格培训,并对已发生压疮者每天访视一次,Braden≤12分的2天访视一次,Braden13-18分,每周访视2次。结果施行延续护理后,护理员对于压疮预防知识及技能的知晓率显著提高。结论实施延续护理加强了托老院护理人员对预防压疮的意识和技能,降低了托老院高危人群压疮的发生率。

Objective To investigate the application feasibility of continuing nursing care in prevention and treatment of pressure sore among high- risk populations in aged care facility. Methods Totally, 282 people from 28 aged care facilities in Taizhou were included in the study conducted from April, 2011 to April, 2013. All participants received the continuing nursing care provided by nursing personnel with rigorous training. People with pressure sores would receive daily visit. And people with Braden score less than 12 would be visited every two days and those with Braden score of 13-18 twice each week. Result After the implementation of continuing nursing care, knowledge and skills of nursing personnel in pressure sore prevention gained significantly improvement. Conclusion The continuing nursing care can strengthen the awareness and skills of nursing personnel in aged care facilities to prevent pressure sores and reduce the incidence among high-risk populations.
