翼状胬肉切除联合角膜缘干细胞移植与联合结膜瓣转移效果观察

Comparing the Effect of Pterygium Combined with Limbal Stem Cell Transplantation and Pterygium Excision with Conjunctival Flap Transfer

目的:探讨翼状胬肉切除术联合结膜瓣移植术与翼状胬肉切除术联合角膜缘干细胞移植术的临床价值。方法:选取我院2008年1月至2012年12月收治的翼状胬肉患者182例共219眼,行翼状胬肉切除术联合结膜瓣转移手术共89例97眼,翼状胬肉切除术联合自体角膜缘干细胞移植手术共93例122眼,对比两组的临床效果。结果:翼状胬肉切除术联合结膜瓣移植创面修复时间相比翼状胬肉切除术联合角膜缘干细胞移植显著延长(P<0.05),且患者流泪与畏光等术后反应较为严重。翼状胬肉切除术联合角膜缘干细胞移植治愈率相比翼状胬肉切除术联合结膜瓣移植显著升高(P<0.05),而复发率显著降低(P<0.05)。结论:翼状胬肉切除术联合角膜缘干细胞移植术治疗翼状胬肉效果较好,治愈率高且复发率低,治疗方法安全有效。

Objective： To investigate the clinical value of pterygium excision with conjunctival flap transplantation and pterygium excision with limbal stem cell transplantation. Method： 182 patients with pterygium with a total of 219 eyes in our hospital from Jan. 2008 to Dec. 2012 were selected,a total of 89 eyes of 97 patients were undergone pterygium excision with conjunctival flap transfer operation; a total of 93 eyes of 122 patients were undergone pterygium excision combined with autologous limbal stem cell transplantation operation,the clinical effects of two groups were compared. Result： Healing time of pterygium excision with conjunctival flap transplantation was significantly longer that of the excision of pterygium combined with limbal stem cell transplantation（ P ＆lt; 0.05）,and patients with tears and photophobia,postoperative symptoms of stress were more serious.Cure rate of excision of pterygium combined with limbal stem cells increased more significantly than that of pterygium excision with conjunctival flap transplantation（ P ＆lt; 0.05）,and the recurrence rate was significantly decreased（ P ＆lt; 0.05）. Conclusion： Excision of pterygium combined with limbal stem cell transplantation has better effect,higher cure rate and lower recurrence rate,it is a safe and effective method for the treatment of pterygium.
