全瓣下结构保留下不同人工瓣膜下游血流动力学指标比较 被引量:7

摘要 目的体内定量研究二尖瓣全瓣下结构保留下人工瓣膜构型对瓣膜下游血流动力学的影响。方法将入选18例患者依据置换的人工瓣膜分为GK-Ⅱ组、St Jude组和HancockⅡ组。检测常规多普勒超声、流场均匀性指标,然后应用软件分析患者术后超声心动图,计算出湍流剪应力和相对扰动强度。采用单因素方差分析比较3组间各指标的差异。结果 3组间二尖瓣平均跨瓣压差(PGmen),二尖瓣跨瓣流量(Qmv),二尖瓣峰值流速(Vmax),峰值流速最大差值(ΔVmax)、外周位点湍流剪应力(TSS)和相对扰动强度(Irel)差异有统计学意义(P<0.05),GK-Ⅱ组最高,HancockⅡ组最低,St Jude组介于两者之间。结论对于心功能较差,有必要保留全瓣下结构者,建议尽量使用生物瓣。
