盐酸氨溴索对颅脑损伤患者的肺保护作用

Lung protection of ambroxol hydrochloride in patients with cerebral trauma

目的探讨盐酸氨溴索对颅脑损伤患者的肺保护作用。方法中重型颅脑损伤患者56例随机分为两组:对照组30例,给予常规颅脑损伤治疗方案;治疗组26例,在常规治疗的基础上加用盐酸氨溴索注射液30mg静脉滴注,每天3次,7d为1个疗程。比较两组肺部并发症相关指标和患者转归。结果治疗组患者出现肺不张、肺部感染、肺水肿及急性呼吸窘迫综合征发生率均低于对照组(P<0.05);治疗组5、7d的肺损伤评分低于对照组(P<0.05);治疗组患者住ICU时间、总住院时间和病死率均低于对照组(P<0.05)。结论盐酸氨溴索注射液在中重型颅脑损伤患者保护肺功能、减少肺部并发症、缩短住院时间和降低病死率方面有一定的临床应用价值。

Objective To explore the lung protection of ambroxol hydrochloride in patients with cerebral trauma. Methods Fifty-six patients with middle to severe cerebral trauma in ICU were randomly assigned into two groups of A （treated with conventional therapy plus ambroxol hydrochloride 30 mg infused tid. for 7 days, 26 cases） and B（treated with conventional therapy alone, 30 cases）. The incidences of pulmonary complication-related indicators and outcomes were recorded and compared between two groups. Results The incidence rates of atelectasis, pulmonary infection, pulmonary edema, and ARDS were significantly lower in group A than those in group B（P〈0. 05）. Lung injury scores on the 5th and 7th day of treatment were significantly less in group A than those in group B（P〈0. 05）. ICU and total hospital stay were shorter in group A than those in group B （P^0. 05）. The mortality was lower in group A than that in group B （P〈0. 05）. Conclusion Ambroxol hydrochloride injection has a certain value in protecting pulmonary function, reducing pulmonary complications,and shortening hospital stay in the patients with middle to severe cerebral traum.
