泮托拉唑与奥曲肽治疗上消化道出血的临床分析

Clinical analysis of pantoprazole and octreotide in treatment of upper gastrointestinal hemorrhage

目的初步探讨泮托拉唑与奥曲肽单独与联合治疗非静脉曲张性上消化道出血的有效性及安全性。方法选取武汉科技大学附属汉阳医院消化内科2009年12月-2012年12月非静脉曲张性消化道出血153例,随机分为A、B、C组,每组51例,在其他对症治疗的基础上,A组单独给予泮托拉唑,B组单独使用奥曲肽,C组泮托拉唑与奥曲肽联合应用。比较3组患者在持续出血时间、出血量、胃液pH值、血红蛋白、临床疗效及不良反应方面的差异。结果 (1)三组患者持续出血时间、输血量、胃液pH值、血红蛋白比较:与A、B组比较,C组持续出血时间、输血量明显减少,胃液pH值、血红蛋白明显升高,差异均有统计学意义(P<0.05)。(2)三组患者临床疗效比较:在显效及总有效率方面,C组均明显高于其他两组,差异均有统计学意义(P<0.05)。(3)不良反应比较:三组患者均未发现明显不良反应。结论对于非静脉曲张性消化道出血,泮托拉唑与奥曲肽联合应用能高效快速止血,应在临床开展应用。

Objective To investigate the effect and safety of pantoprazole and octreotide alone and combination therapy in the treatment of non variceal upper gastrointestinal hemorrhage. Methods From Dec. 2009 to Dec. 2012, 153 cases of non variceal gastrointestinal hemorrhage were randomly divided into A, B, C group. Based on other symptomatic treatment, A group received pantoprazole, B group received alone octreotide, C group were given pantoprazole combined with octreotide. Persistent bleeding time, bleeding volume, the pH value of gastric juice, hemoglobin, clinical curative effect and adverse reaction were compared in three groups. Results （ 1 ） Compared with A, B groups, continuous bleeding time, volume of blood transfusion were significantly reduced, and the pH value of gastric juice, hemoglobin were increased in C group, there were statistically significant differences （P 〈 0.05）. （2） The effect and the total efficiency were significantly higher in C group than the other two groups, there was significant difference （P 〈 0.05）. （3） No obvious adverse reactions were found in three groups. Conclusion For non variceal hemorrhage of digestive tract, pantoprazole and octreotide can fast stop bleeding, should be carried out in the clinical application.
