血必净注射液对心肺复苏后患者血流动力学及组织灌注的影响

Effect of Xuebijing injection on haemodynamics and tissue perfusion of the patients after cardiopulmonary resuscitation

目的观察血必净注射液干预对心搏骤停、心肺复苏后患者血流动力学及组织灌注的影响。方法 86例心搏骤停经心肺复苏恢复自主循环的患者随机分为两组,对照组43例给予循环、呼吸支持、脑保护以及防治感染等常规治疗措施;治疗组43例在常规治疗基础上加用血必净注射液100ml加入0.9%生理盐水100ml中静脉点滴,每天2次,疗程2周。观察两组干预后14天的死亡率,治疗前、后急性生理与慢性健康(APACHEⅡ)评分,血流动力学指标心脏排血指数(CI)、平均动脉压(MAP)、肺血管通透指数(PVPI)、外周血管阻力(SVRI)以及血乳酸(LAC)的变化。结果与对照组比较,治疗组的14天死亡率有所下降,APACHEⅡ评分、血乳酸水平及PVPI、SVRI均明显降低,而CI、MAP则较之升高(均P<0.01)。结论血必净注射液能改善心肺复苏后患者的血流动力学及组织灌注。

Objective To observe the therapeutic effect of Xuebijing injection on haemodynamics and tissue perfu- sion of the patients after eardiopulmonary resuscitation. Methods 86 patients after cardiopulmonary resuscitation were randomly divided into the treated group （n=43） and the control group （n=43）. The patients in control group were given mild hypothermia to preserve the function of brain, ventilator to help breathing, to protect the function of other organs, antibiotics to prevention and cure infection, nutritional support and so on. While the patients in the treated group were accepted Xuebijing injection 100ml twice daily except for the conventional therapy. The APACHE Ⅱ score, 14 day mor- tality rate, cardiac output index （CI）, mean arterial pressure （MAP）,pulmonary vascular permeability index（PVPI）, systemic vascular resistance index （SVRI） and blood lactic acid were determined. Results Compared with the control group, the 14 day mortality rate was lower in the patients of treated group. APACHE II score, blood lactic acid, PVPI and SVRI of the patients in treated group was much lower than that of before, while CI and MAP was much higher（P〈 0.01）. Conclusion Xuebijing injection can improve haemodynamics and tissue perfusion of the patients after cardiopul- monary resuscitation.
